**about the role**:
The Community Resource Worker (CRW) provides direct support to clients and indirect support to the Primary Care Team in order to facilitate navigation of the health and social service systems within Sherbourne Health and the broader community.

**scope of the mandate**:
The Community Resource Worker (CRW) locates, navigates, and creates appropriate linkages to health and social resources for the Primary Care clients and potential clients.

The CRW works as a member of an interdisciplinary team and provides information, referral, education, support and follow-up services to Primary Care clients and potential clients.

The CRW maintains accurate and up-to-date information regarding available community resources, develops working relationships with community resource providers and develops in-house resources to address common barriers (e.g., health card renewals).

**day to day responsibilities**:
**Direct Service (engaging assessing, planning, system navigation and follow-up)**
- Develops a respectful, client-centered relationship through referral and/or outreach.
- Identifies client strengths, abilities, preferences, resources, needs and formal and informal supports.
- Provides information about community resources in a form that facilitates use by clients (e.g., plain language, culturally appropriate)
- Supports clients to identify and access needed community resources, as appropriate, in collaboration with clients and the interdisciplinary team. This may include formal and informal resources and the full range of community services (i.e., health, social, educational, vocational, legal, and spiritual).
- Assists clients to obtain harm reduction and health promotion resources
- Navigates client access to resources using referral, direct support (e.g., accompaniment), resource arrangement (e.g., interpretation services) and advocacy on behalf of clients, with appropriate client consent.
- Provides follow-up and problem-resolution to ensure connections with community resources have taken place.
- Works closely in partnership with clients, the interdisciplinary team, and other programs and services within Sherbourne and the community to ensure clients are able to navigate services.
- Works with the interdisciplinary team to intervene with clients in crisis
- Participates actively in interdisciplinary case conferences
- Provides opportunities for clients to evaluate their progress and make choices regarding community resource options
- Communicates effectively with other team members/providers to ensure optimum care within the boundaries of appropriate confidentiality and disclosure.
- Seeks consultation and/or supervision as needed
- Facilitates or arranges groups or workshops in partnership with Sherbourne staff or other community organizations (e.g., housing information forums)
