Clinical Lpn Lead

**Position Overview**: The Clinical Lead oversees clinical staff, ensuring high standards of care and managing daily operations. Responsibilities include staffing, admissions, discharges, transfers, and supporting hospice and home care clients. The role involves quality monitoring, audits, training, policy reviews, and ensuring compliance with education and safety protocols.

**Key Responsibilities**:
- **Staff Support and Coordination**:
- Supervise and support clinical staff on the floor.
- Provide coverage for staffing shortages and physician rounds if needed.
- Conduct performance and attendance reviews, and manage disciplinary processes.
- **Quality and Compliance Monitoring**:
- Conduct spot checks on documentation, MAR sign-offs, PRNs, and bath/shower temperatures.
- Prepare for and review audits (AUA, Falls Rounds).
- Audit care plans and update new orders.
- Manage Lifeline database and conduct quarterly chart audits.
- Prepare for and lead care conferences; manage wound care supplies inventory.
- Review acuity tracker data and attend relevant meetings.
- **Resident and Family Engagement**:
- Review and document Goals of Care with decision-makers; review annually.
- Ensure pendant checks and review response times.
- Address resident and family concerns; assist with admissions, discharges, transfers, and home care coordination.
- **Education and Training**:
- Provide on-the-job education and work with the clinical educator.
- Monitor and ensure compliance with mandatory education.
- Lead monthly Code of the Month reviews and IPC education.
- **Outbreak Management and Safety**:
- Support the DOC during outbreaks, ensure PPE stock, and update policies.
- **Administrative Duties**:
- Update case managers on care needs; maintain external partner relationships.
- Support scheduling and monitor staff certifications and performance.
- Review policies, maintain sign sheets, and attend various meetings.
**Bona-fide Requirements**:
- Share clinical on-call responsibilities.
- Ability to lift 10+ pounds.
- Obtain and maintain a criminal record check with vulnerable sector screening.
- Maintain compliance with all required certifications and training.
- Ability to handle multiple priorities and work effectively under pressure.
**Qualifications**:
- RN or LPN with relevant clinical experience.
- Demonstrated skills in leadership, communication, teamwork, concern resolution, and customer service.
- Strong organizational and computer skills.
- Ability to problem-solve through critical thinking and self-education.
- Enthusiastic attitude and energy to provide excellent care and service.
- Awareness of accident and injury prevention, hazard identification, and control; adherence to safe work practices.
- Demonstrated knowledge and commitment to person-centered care.
