**NOW HIRING: Health System Navigator.**
**JOB SUMMARY** - as part of a multi-disciplinary health team and using a co-ordinated care approach(as with HealthLinks), the Navigator is responsible for assessing each client, creating and facilitating a Coordinated Care Plan, and then identifying and connecting the client with appropriate health and social services that will build individual resilience while maintaining independence. The Navigator also assists in the development of skills and roles which promote meaningful participation in the typical and valued life of the community. This approach requires working collaboratively with health care professionals, health and social service organizations and directly with the client and their support network.
Each Navigator brings a unique skill set that is enhanced when combined with other health professionals in a multidisciplinary team.

- _**an Occupational Therapist (E/Fr Bilingual); and**_
- _**a Registered Social Service Worker (E/Fr Bilingual).**_
**DUTIES & RESPONSIBILITIES** - focusing on quality client service using sound judgment, professionalism and effective communication, the Navigator provides
**Client Support Functions, including**:
- Screening and Assessment
- using the interrai-CHA suite of tools and others as needed;
- health & social system transition
- Care Co-ordination
- providing in-home assessment to identify client care needs, interests and abilities through a person-centred planning process;
- develop a written Coordinated Care Plan (CCP) in partnership with each client;
- advocate for access to appropriate services and supports as needed by the client;
- identify and liaise with the client’s natural supports, building on these relationships to optimize client independence;
- engage the client and support network in discharge planning.
- System Navigation.
- Education:
- improve self-management skills through health education and information;
- support individuals to develop decision-making skills that support positive self-care, personal responsibilities, good choices, and management of reasonable risks;
- support, educate and connect clients in all areas of daily living including but not limited to meal preparation, shopping, hygiene, personal care, budgeting, safety awareness, and maintaining a healthy living environment.
- Support Effective Risk Management, and Connection with community.
- **QUALIFICATIONS, SKILLS & REQUIREMENTS**Education - current registration with a professional college in Ontario.
Experience - practical experience working in a community-based setting is preferred.

- able to walk, stand, bend, squat, kneel, climb stairs and reach freely;
- Clear police record check for the vulnerable sector
