Health and Wellness Manager

**Position Overview**
Health and Wellness Manager reports to the General Manager and is responsible for providing direction to the nursing team in regards to the overall day-to-day delivery of healthcare operations, ensuring that the healthcare services comply with the Retirement Homes Act (RHA) and other relevant legislation.
**Responsibilities**
- Assigns, delegates and evaluates the activities of the nursing department Provide professional nursing care to all residents and supervise care given in the nursing unit,
- Plans, assigns and overseas the Residents’ health and personal care services,
- Participates in inter-professional assessments in identifying Residents’ needs and develops an individualized plan of care, ensuring that the resident is receiving the level and quality of agreed upon care,
- Assesses all resident needs and planning, implementing and evaluating the care program,
- Assesses residents' conditions by recognizing signs and symptoms and where necessary initiates remedial health measures,
- Implements and monitors quality improvement,
- Meets with Residents/Families to resolve problems and concerns,
- Organizes and participates in the Resident care conference on admission and at 6 monthly intervals or as the condition of the Resident changes,
- Maintains accurate and complete health records,
- Contacts resident's attending physician when there is a change in resident's physical or mental condition,
- Responsible to ensure safekeeping of drug and narcotic keys,
- Oversees the medication administration program,
- Reviews pre-medical assessments to ensure that the community is able to provide the care,
- Serves as the Infection Prevention and Control Coordinator for the community,
- Responsible for the recruitment of associates,
- Completes performance appraisals of associates as required Monitors staff performance and completes performance appraisals during probation period and annually thereafter,
- Responsible for leadership, coaching, and mentoring to all regulated and Unregulated Care Providers,
- Responsible for admission and discharge of residents,
- Teaches good health habits and deals courteously with staff, residents, residents' families and visitors,
- Maintains liaison with families of residents,
- Provides ongoing associate education,
- Maintains the staffing schedules,
- Promotes the Vision, Mission and Values of Lifetimes Living Inc. and the Resident Bill of Rights,
- Takes prospective residents and other interested persons on tours of the retirement residence,
- Ensures medical records are maintained,
- Ensures all personal health information is maintained as per PHIPA,
- Ensures daily/weekly and monthly duties are completed and rosters are maintained,
- Organizing and ensuring medications are reviewed in keeping with the residence’s medication review cycle, where medications are provided,
- Completes monthly weights and blood pressures on all residents,
- Coordinates and/or participates in the staff development, quality assurance, safety, environment and infection control programs,
- Ensures staff involvement in fire and safety procedures and communication to all staff of inspection reports and follow up actions,
- Ensures Information Package is distributed to all new admissions.
- Knowledgeable of the Residential Tenancies Act,
- Initiate review of Resident Care policy and procedure as per the designated review schedule and as required,
- Ensures nursing documentation is complete as per College of Nurses' Nursing Documentation guidelines,
- Assists with arranging transportation for residents,
- Answers call bells and assist where necessary,
- Lead/initiate/monitor emergency response protocol and effectiveness for the department,
- Orders medications from pharmacy as per Doctor's orders,
- Receives medications and ensures safe storage of medications,
- Serves and supervises at meals - assist with resident satisfaction,
- Adheres to all established policies and procedures,
- Responsible for making the appropriate plans for discharge as necessary,
- Completes and follows up on incident reports where necessary,
- Supervises resident's personal hygiene and bathing as per schedule, or assisting when necessary,
- Assists residents with ambulation when necessary,
- Provides emergency medical care,
- Responsible for maintaining an accurate waiting list,
- Responsible for maintaining accurate records for "extra charges" if applicable,
- Responsible for ensuring and arranging that all residents have an annual physical,
- Ensures there are sufficient staff on duty to provide the services which are offered,
- Assists residents and/or families in obtaining the services of a physician when necessary,
- Ensures a resident care plan is established and reviewed/revised at least every six months,
- Accesses, facilitates and orientates community agencies and/or resources,
- Facilitates contact with other Health Care facilities and educational facilities,
- Collaborates with volunteer
