**Full time Care Coordinator ; Job Location - Peterborough**
The Care Coordinator is responsible for ensuring a positive client and family experience through effectively scheduling direct care providers in the community to meet the requirements of client care plans. Care Coordinators work diligently to facilitate continuity of care while meeting the scheduling requirements of the clients and care providers. Frequently problem-solving, they are the central point of contact for clients and families, care providers and referral sources. Effective care coordination is an important element of service delivery. Individuals in this role must be driven to help others in a compassionate and caring way while balancing the pressure to deliver on scheduling requirements.
**KEY RESPONSIBILITIES/ACCOUNTABILITIES**
- Utilize scheduling software to ensure client care plans are fulfilled by scheduling care providers effectively and efficiently
- Hitting individual KPIs (calls per hour, quality, and filling visits)
- Communicate effectively with clients, families, and care providers to ensure schedules are carried out. Problem solves issues efficiently and escalate as required
- Pursue and maintain positive relationships with care providers, clients and funders. Seek to understand individual needs and consider those needs in decision making
- Maintain accurate information within databases such as scheduling and availability changes, care plans, and client records
- Enter all the critical and relevant information into the system(s) for reporting and tracking purposes
- Adhere to Employment Standards legislation, collective agreements and/or policies and procedures
**KNOWLEDGE, EDUCATION AND EXPERIENCE**
- Minimum Grade 12 education
- Minimum 2 years of experience in Customer Service, Scheduling and/or Coordination
- Computer skills - Microsoft Office Suite, ability to use and utilize scheduling system (AlayaCare)
**TECHNICAL AND INTERPERSONAL SKILLS**
- Strong interpersonal skills
- Results-oriented with problem-solving skills
- Strong data entry and analytical skills
- Ability to multi-task in a fast-paced environment
- Demonstrate excellent telephone etiquette
- Effective communication skills both verbally and in writing
- Excellent time management and organizational skills
- Excellent listening and observational skills
**OTHER CONSIDERATIONS**
- 2 years of experience scheduling in a call center environment or team focused on service delivery
- Medical terminology and/or medical administration background considered an asset
- **Ability to communicate in French is considered an asset**:
- Understanding of employment legislation is considered an asset
- Scheduling experience preferably in the health care field is considered an asset
- Be able to work a **Full-time Monday to Friday 8:30am to 4:30pm** work schedule in office
