We are seeking a **Care Coordinator, PT **who will be the unique bridge between our administrative/clinical teams, and direct care to the young women we support. This role may be ideal for an individual who wishes to pursue part time education or upgrading, as you are expected to work with a flexibility to self-schedule based on young person/program needs.
The Care Coordinator will:
- Be an **active contributor to the Referral and Intake processes** of the program, providing review and assessment of all documentation and preparing both the Safety and Needs Reports (Quality Standards Framework, 2021) to the Admin/Clinical team to aid with suitability matches.
- Provide **mentorship** and, on occasion direct coaching, to the front-line team to assist with relational care approaches, development of Safety plans, IR debriefing, Extra Care, Healing plans and Transitional care
- Actively **engage in community networks, social media, job fairs** and other activities to promote the program and to ensure that youth voices are shared and heard.
- Support the work of the direct care team (DCF & Team Leaders), through **contribution to House meetings, special activities **and direct supports of children/youth in the program.
- **Assist** the work of the Leadership team, through **contribution to program evaluation, team meetings, advocacy efforts,** community partnership and leadership meetings.

- Salary rate of $**31.86 hourly plus** compensation in lieu of vacation (5%) and extended health benefits (8%) paid bi-weekly

- Identify and communicate systemic barriers impacting the young person and assist in advocating for barrier mediation and/or elimination
- Work as part of a multi-disciplinary team of professionals, which include Director, supervisor, and clinical care team (e.g. social workers, nurses), all with the aim of ensuring best care practices are provided to the young person
- Have a valid G driver’s license, clean driving abstract, reliable vehicle & willingness to travel to work site located in Peterborough County
- Provide proof of Immunization records, must have physician’s note (fitness to work)
- Provide a current Criminal Reference check, including Vulnerable Sector Screening
- Have proof of Post-Secondary education. A minimum of level of education required is a Bachelor’s degree, with a preference for those with a Child and Youth care designation **or** a minimum level of education in a 2 year diploma in a related field (social services diploma, mental health & addictions, police foundations as examples) _paired_ with direct experience in the sector
