**Position Title: Social Worker**

**About the Position**
- Discharge planning for complex discharges
- Providing resources and crisis intervention to patients and families
- Navigate POA/SDM issues with
- Work with PGT as needed in complex cases
- Take lead on the coordination of MAiD cases
- Navigate addiction, homelessness and family dynamic concerns
- Contribute to interventions for patients with responsive behaviours
- Serve as a resource/consultant to the interdisciplinary health care team.
- Facilitate communication and understanding between patient, family and treatment team in critical and non-critical patient care situations.
- Serve as Chair of the Ethics Committee and consultant for ethical issues.
- Complete all required documentation and data collection

**Qualifications**:

- Master’s Degree in Social Work (MSW).
- Active member in good standing of the Ontario College of Certified Social Workers and Social Service Workers.
- Minimum two (2) years’ experience in a hospital setting (preferred).
- Social work experience in Intensive Care, Palliative Care, End of Life, Substance Abuse, Homelessness, Community Resources, Emergency, Psychiatry and Patient Placement and Flow.
- Demonstrated interest and ability to balance a diverse caseload.
- Ability to work under pressure, maintaining steadiness, composure and professional disposition.
- Ability to work without direct supervision.
- Ability to work collaboratively, flexibly and in partnership with multidisciplinary teams and agencies in the community and the broader health care system.
- Excellent interpersonal skills, organizational skills, and clinical acumen.
- Ability to use computer programs for data entry of assessments, etc.
- Adherence to all health and safety policies, procedures and provisions and the recognition and reporting of unsafe hazards and work practices

Our area of focus provides a continuum of service throughout the life span of the people we serve including Emergency, Critical Care, Obstetrics, Pediatrics, Medicine, Surgery, and Complex Continuing Care. Employing approximately 550 highly skilled and dedicated employees, Norfolk General Hospital is a full service 106-bed hospital with round the clock on-call coverage of Specialists in Surgery, Anesthesia, Internal Medicine, and Obstetrics, as well as on-site 24 hour per day medical coverage.

**Hours of Work**:Monday to Friday, but must be available to work shifts, weekends and holidays as required.

Pay: $44.67-$57.90 per hour

Expected hours: 22.5 per week
