**Position Summary**

In the Patient’s Medical Home, continuous patient care is provided and led by the family physicians in collaboration with other healthcare providers in the community, specialty, and acute care. The Program Manager - Medical Home Support reports to the Senior Manager, Member Services, and directly oversees the performance management and operational functions of the Practice Facilitators and EMR Support teams.

These teams work directly with members and clinics to implement, monitor, track, report, and evaluate quality improvement initiatives for patient health outcomes through the Medical Home Support Program (also known as “the Program”). This role oversees all aspects of the operations of the Medical Home Support Program to ensure patient outcomes are met, and support members and medical teams to achieve excellence in their unique Medical Home practices with the right resources at the right time for the right primary care outcomes.

**Key Responsibilities**

Program Development & Evaluation
- Lead the planning, development, operation and monitoring of the assigned program areas and initiatives (projects, education, events) aimed at supporting inter-professional practice, role optimization, and improved patient outcomes through design and systems thinking and change management principles.
- Facilitate the development of localized strategies to achieve the Patient's Medical Home implementation within Mosaic PCN membership, in alignment and coordination, as required, with Alberta Health Services (AHS), AMA, HQCA, internal departments, and other external partners.
- Responsible for supporting and leading the implementation, evaluation, tracking, analyses, and reporting of the Medical Home Support Program to meet program, divisional, departmental, and organizational outcomes and targets to the Senior Manager, Member Services.
- Represent Mosaic PCN on identified working groups, as required. Present to working groups and/or or internal departments the Medical Home Support Program and their operational functions in collaboration and the direction of the Senior Manager, Member Services.
- Work with the Senior Manager, Member Services, and other departmental and divisional managers, in developing recommendations, options, analyses, and reports to move forward improvement initiatives and projects as delegated.
- Develop pan-system collaborative working relationships with other improvement stakeholder teams to harness collective capacity for knowledge sharing and
- Overall monitoring, tracking, and reporting of member compliance in all agreements; responsible for ensuring the follow-up of un-signed agreements; Work closely with the Manager, Member Administration to ensure overall adherence to service level agreements, issues management, and work with members’ team(s) to ensure compliance as required.
- Work with the Senior Manager, Member Services, to develop, document, track, report, and improve operational escalation processes and communications, and all other organizational and departmental initiatives as assigned.
- Responsible for developing, updating, tracking, and reporting change management plans and initiatives, including all forms of communications to support the change management plan, including tracking of metrics and recommending new ones, or deleting old ones, as required.
- Oversee the development, updating, and monitoring of operational best practice program standards, policies (if any), processes, and procedures, including the development and maintenance of the Program’s Standard Operating Procedures (SOP) to reflect current standards and best practices, and adherence to all relevant agreements and governmental requirements.
- Responsible for overseeing the roll-out, implementation, tracking, monitoring, reporting, and staff adherence to all internal organizational and program documentation and agreements.
