**Job Summary**:
The Health Planner is responsible for planning and evaluation support within Approaches to Community Wellbeing (ACW) on programming and special projects, as well as supporting Tribal Council and community-level public health planning.

The Health Planner is directly accountable to the Roots for Community Wellbeing Manager.

**Qualifications**:

- Persons of First Nations ancestry will be given preference (OHRC, Part II, Special Employment)
- 2 years direct work experience in a project management capacity, including all aspects of process development and execution
- Training and/or experience in health planning and evaluation
- Understanding of public health systems, and First Nations health systems
- Experience at working both independently and in a team-orientated, collaborative environment is essential
- Strong written and oral communication skills
- Strong leadership skills, supervisory skills and facilitating skills
- Ability to establish relationships/partnerships with agencies & community
- Must be willing to travel
- Ability to communicate in one of the First Nations dialects within the Sioux Lookout Zone an asset
- Certification in project management is an asset
- First Nations Health Managers Association certification is an asset
- The ability to perform the requirements of the role on a regular basis.

**Roles and Responsibilities**:

- Support the ongoing planning for programs and services within Approaches to Community Wellbeing
- Identify service gaps, community needs, and priorities;
- Develop planned approaches to meet community needs and priorities.
- Develop, implement, and enhance community engagement and relationship building to engage partners in addressing health inequities and keeping them apprised of progress.
- Conduct community consultation processes such as community visits, meetings, conferences, video conferences, and teleconferences to gain input into the model.
- Regularly collect, compile, and analyze data, and generate reports on the progress of ACW annual work plan implementation.
- Understand and facilitate the research and evaluation process to support the development of Approaches to Community Wellbeing.
- Work in consultation with relevant working groups and advisory committees, the Public Health Physician, Tribal Councils and communities to support the development of the regional model, including a transition and implementation plan.
- Share progress and seek guidance from the Health Services Advisory Committee on program planning.
- Host and facilitate annual Health Directors meetings to inform Health Directors of project/planning progress and garner input into programming.
- Support the writing of reports and proposals, as necessary, to support the development of regional Approaches to Community Wellbeing.
- Support the transition process for the Approaches to Community Wellbeing, by seeking additional funding sources, conducting further planning and stakeholder engagement, and supporting a change management strategy.
- Support the Community Wellbeing Facilitators in their training, planning, and community engagement processes. Act as a mentor and resource to Community Wellbeing Facilitators.
- Oversee the development of contribution agreements for Tribal Councils and communities in support of the Community Wellbeing Facilitators program and ensure reporting requirements are met.
- Identify community health priorities, support the strategic health planning function of ACW, and facilitate the continued implementation of ACW and the Anishinabe Health Plan
- Includes developing situational/needs assessments, conducting and facilitating focus groups, etc.;
- Collect and analyze data, along with the Epidemiologist from various data sources to inform the development of project, programs, and services.
- Develop methods to ensure the dissemination, uptake, and utilization of program evaluation recommendations, working with the Knowledge Translation Specialist.
- Support Approaches to Community Wellbeing staff, Tribal Councils, and communities in planning and evaluation.
- Develop and share best practices, tools, resources, and frameworks as appropriate
- Facilitate learning and training sessions to improve capacity in effective public health practice for Approaches to Community Wellbeing staff, Tribal Councils, and communities through conducting internal evaluations.
- Supporting performance measurement, monitoring, and continuous quality improvement of Approaches to Community Wellbeing to ensure activities address departmental needs and priorities.
