Reporting to the Manager, the Social Service Worker (SSW) is a member of an interdisciplinary healthcare team providing assistance in the assessment of the social service needs and resources for individuals and families. This will include completing assessments and liaising with social work and mental health and addiction colleagues and community partners. Responsible for problem solving and conflict resolution skills to develop effective solutions to issues including crisis situations, family conflict or other interpersonal situations that arise. The role will involve aspects of patient care associated with triage and clinical interventions in accordance to practice guidelines. This position contributes to the shared responsibility of ensuring health and safety policies are followed to provide for a healthy environment for residents, staff, physicians, volunteers and families.

Rate of Pay: $24.09 per hour - $30.12 per hour

Hours of Work: 37.5 hours per week

**Qualifications**:

- 2 Year Social Services Worker Diploma
- Current registration with the Ontario College of Social Workers and Social Service Workers
- Minimum six (6) months recent clinical experience in social services or hospital setting
- Demonstrated knowledge of relevant legislation in relation to the health care consent act and substitute decisions act
- Demonstrated strong critical thinking and problem-solving skills
- Demonstrated proficiency in Microsoft Office and familiarity with electronic health records
- Possesses strong communication skills (both written and verbal)
- Possesses strong presentation, interpersonal and facilitation skills
- Demonstrated practice and commitment to the principles of patient and family centered care
- Demonstrated practice and commitment to patient and staff safety at LHSC
- Demonstrated practice and commitment to LHSC’s Mission, Vision and Values
- Demonstrated ability to attend work on a regular basis

**Immunization Requirements**:

- Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox), Hepatitis B, Tetanus/ Diphtheria/Polio; Meningitis.
- Provide documentation of the Tuberculosis skin testing (two step) and COVID vaccine series.
