Carleton Place, Canada ConnectWell Community Health Full time

Manage a caseload of approximately 80 families of children, youth & adults with special needs ensuring their recreational, respite and funding needs are met
- Develop and monitor a budget based on allocated funds and family needs for each client
- Available to provide direct care support within program settings
- Ability to work a flexible schedule as the position will involve evening & weekend work
- Ability to participate in an afterhours on call rotation

**Requirements for this position include**:
**Education**:

- Diploma or Degree in one of the Social Sciences

**Professional Experience**:

- Substantial and proven experience in working with children/youth & adults with special needs. As well as experience working within a respite/share environment & Coordinated Service planning model.

**Knowledge, Skills and Abilities**:

- Knowledge of child & adult provincial funding
- Experience in Case Management
- Skills in assessing and creating community linkages
- Experience with coordinating respite services
- Experience with programming
- Excellent oral, written and organizational skills
- Flexibility in completion of job related tasks

**Reporting Relationship**:
The Caseworker reports directly to the Respite Services Intake & Casework Manager.

**Salary**: $28.00-$31.00 per hour
