**Job Summary**:
This RDFP Program Lead reports to the Executive Director, Richmond Division of Family Practice and following the direction set by the Primary Care Network Steering Committee, the Primary Care Network (PCN). The Lead is responsible for operationalizing and project management of both RDFP and PCN initiatives within Richmond and is the liaison to the Program Manager and other stakeholders. The Program Lead performs a variety of tasks including, but not limited to, leading projects, coordinating resources, facilitating planning, setting deadlines, monitoring, gathering data, evaluating, summarizing, and communicating the progress of this initiative within a quality framework.

**Key Accountabilities and Responsibilities**

Sets strategic objectives with the Executive Director and Program Manager that align with other Division priorities.

Develops and activates plans and communication materials to recruit Family Physicians and Nurse Practitioners to PCN contract positions.
- Develops and implements recruitment strategies to PCN contract positions
- Oversees development of recruitment to Richmond Division website
- Coordinates work with RDFP PMH Lead on alignment of recruitment plans and activities
- Engages with RDFP member clinic physicians and staff
- Ensures all work within the PCN aligns with the quality framework, and will provide data as evidence of this success

Plan work, allocate resources, define tasks, and assign responsibility among PCN administrative staff to ensure systems are efficient, effective, and appropriately documented in support of Division physician members, and patients.
- Oversee day-day issue management and workflow related to PCN administrative systems, to ensure alignment with clinical processes
- People management: from recruitment to performance evaluation, monitoring team well-being (including vacation, sick time, work exceptions etc.) actively seek to develop and maintain meaningful work.

Coordinate and lead the Expression of Interest (EOI) and selection process to place FP/NP resources into host clinics.
- EOI process; clinic review; discussions with hosts and resources regarding contract deliverables, recommendation to PCN Steering Committee and communication with practice and stakeholders.
- Document a transparent and data driven selection and assignment protocol for FP/NP allocation to clinics.

Provide overall management for the Attachment Program to ensure accuracy and efficiency.
- Ensures functionality of program including Rapid Access Program (RAP) and Health Connect Registry (HCR) database, data cleaning and accuracy within spreadsheets. Works with Communications resource to address changes to HCR page and RDFP website, patient and provider materials regarding attachment program.
- Point of contact for Ministry of Health regarding Health Connect Registry; link to Communications resources to develop communication needs for Health Connect Registry

Support the strategic objectives and plans to integrate primary care practices with PCN team.
- Supports implementation effort with activities between Primary Care Network and Patient Medical Home programs
- Participate in Division events

Collects data and provides analysis to ensure the completion and submission of various reports.
- Supports the completion and filing of monthly, quarterly, and ad hoc reports for Ministry of Health and as needed reports for RDFP Board of Directors and VCH stakeholders

Support the PCN Steering Committee.
- Supports the steering committee with agenda setting, creation of reports and briefing of the VCH Director of Primary Care and RDFP Executive Director in advance of the meeting.

Ensures FP and NP consultation by:

- Developing relationships with clinics and staff for the FP leadership group
- Consultation and engagement activities with FPs/NPs
- Bringing forward FP/NP perspectives to planning and stakeholders

General:

- Supports other RDFP or PCN initiatives as identified by the RDFP Executive Director Board of Directors or Steering Committee,
- Develops and documents policies, processes, workflows, and procedures.
- Acts as an Ambassador of the Richmond Division of Family Practice and Richmond Primary Care Network, modeling professionalism at every opportunity.

**Working Conditions**
- Full-time, office-based on Monday and Fridays, RDFP office Tuesday through Thursday.
- Hours: Monday-Friday, 8:30 am - 5 pm business hours with some early morning and evening meetings, as required
- Ability to travel to locations primarily in Richmond
