**Description**

**One.5 FTE Contract to March 31, 2025 (17.5** hrs/week) **(with the possibility of extension or becoming permanent)

**Dual Diagnosis Case Manager** - The Mental Health Dual Diagnosis Case Manager is responsible for the provision of comprehensive case management and advocacy services to individuals age sixteen and over who are experiencing significant difficulties in daily functioning as a result of serious mental illness and developmental disability.

The Mental Health Dual Diagnosis Case Manager collaborates with consumers, community partners, and the general public in response to individually identified goals, with a focus on increasing the quality of life for individuals served. In addition, the Mental Health Dual Diagnosis Case Manager participates in the development of community resources and creative strategies to address service system issues and public education and awareness.

**Position Requirements**
- Bachelors degree in Social Work or related discipline (i.e. Health Sciences)
- Minimum 3 years related experience required
- Cross sectorial systems knowledge for mental health and developmental services
- Francophone or second language is considered an asset
- Knowledge and practice of psychosocial rehabilitation approach and advanced clinical and assessment skills
- Counselling, crisis prevention and intervention experience
- Experience working with individuals having serious mental illness and a developmental disability
- Experience in working with a multi-disciplinary team
- Meet clients face-to-face - in their homes, in our offices, and in the community
- A valid G Drivers License, access to a reliable vehicle and business use insurance coverage, OR access to reliable transportation.

**Office Hours**:Monday - Friday - 8:30 am - 4:30 pm
**Office Expectations**: In office once per month, as well as for team coverage, meetings, and trainings when required.
