**SCOPE**:
The **Ska-Be** **Personal Support Worker **will be responsible to provide personal care services to Indigenous Community Members with the activities of daily living as described in the care plan, in accordance with all required internal and external standards of practice with a community member led care focus.

The Indigenous Ska-Be program will provide support services to improve the health and well-being of urban Indigenous seniors and Indigenous persons with disabilities, with a particular focus on Indigenous women, to assist them in achieving a good quality of life that is reflective of their needs and their culture. Ska-be will offer education, hands-on support services to assist Community Members with activities of daily living, mental health needs and follow up in a traditional continuum of care model. The Ska-Be worker will ensure that Community Members are supported in their home environments and assisted in their awareness and practice of traditions, culture, and ceremony as needed. The Ska-Be worker will provide access to culturally relevant programming and culturally relevant navigation of all health systems and appointments.

**QUALIFICATIONS**:

- Personal Support Worker Certificate (or other caregiver certification will be considered), along with a minimum of 2 years of care giving experience with geriatrics, long-term care or home services required.
- Strong knowledge of the principles, practices and methods of community nursing and the delivery of personal care service to Community Members.
- Knowledge of and ability to navigate health services within service areas and across Ontario.
- Able to stand, bend, squat, kneel and reach freely.
- Able to freely lift to fifty (50) pounds.
- Able to assist Community Members with standing, walking, sitting, bathing, cooking, light cleaning, attending appointments etc.
- Able to provide basic wound care and other PSW related health care services.
- Able to cook healthy meals as well as perform cleaning duties.
- Demonstrated experience operating related equipment.
- Excellent oral and written communication skills.
- Experience working on a collaborative team.
- Able to manage many conflicting priorities simultaneously, with the ability to prioritize.
- Strong connection to other social service providers within the community.
- Knowledge of not-for-profit Indigenous organizations.
- Demonstrated ability to work independently with mínimal supervision.
- Strong organizational/time management skills with an ability to plan ahead and define measurable objectives and outcomes and meet objectives in a timely manner.
- Working knowledge of MS Office Software, internet, and general office equipment.
- Valid G Class Driver’s License and access to a reliable vehicle with appropriate insurance.
- Current, valid vulnerable sector criminal records. Valid 1st Aid/CPR.
- A cellular phone and internet access is required within this position.
- Must be willing and able to work a flexible working from home arrangement if This may include a combination of virtual remote work in your home and in office arrangements. This will be based on operational needs and will be determined by the ONWA.
- Ability/willingness to travel and to work flexible hours.
- Dependable and punctual.
- Able to write and document detailed case notes and collect statistics.
