**Overview**

The role is flexible and can be an extra hours for those who would like, permanent full time or part time.

**Duties**
- Conduct thorough patient assessments and evaluations, including medical history and physical examinations.
- Develop and implement individualized treatment plans based on patient needs and preferences.
- Provide ongoing management of chronic conditions and acute illnesses, ensuring continuity of care.
- Perform minor procedures as necessary, including pain management techniques.
- Collaborate with other healthcare professionals to coordinate patient care effectively.
- Maintain accurate and up-to-date medical records using electronic health record systems such as Epic or Cerner.
- Educate patients and their families about health maintenance, disease prevention, and treatment options.
- Participate in data collection for quality improvement initiatives and research projects as needed.
- Provision of a special interest clinic as agreed

**Experience**
- Board certification in Family Medicine or eligibility for certification is required.
- Experience in home care settings is a plus, along with familiarity with medical terminology relevant to the field.
- Proficiency in using electronic health record systems Med Access is highly desirable.
- Strong interpersonal skills with a focus on patient care and satisfaction are essential for success in this role.

Pay: $140.00-$180.00 per hour

Expected hours: 4 - 40 per week
