POSITION: Programs & Community Outreach Coordinator

4 Month Contract, with possibility to extend

REPORTS TO: Executive Director

Position Summary: The Programs & Community Outreach Coordinator plays a pivotal role in providing comprehensive support to clients by spearheading the leadership and coordination of our daily programs. This position also focuses on promoting awareness of Hospice Georgina and nurturing relationships with local community groups and organizations to bolster awareness of Hospice Georgina's services.

DUTIES AND RESPONSIBILITIES:
**Client Service Excellence**:

- Ensure cost-effective delivery of client services and programs.
- Assess service needs, monitor delivery, and contribute to program development, evaluation, and promotion.
- Develop and execute program plans aligned with departmental needs.

**Community Engagement and Relations**:

- Maintain consistent engagement with clients and partners to align programs with community needs.
- Uphold financial accountability by managing program budgets effectively.
- Encourage increased community involvement to attract additional volunteers.

**External and Internal Collaborations**:

- Communicate promptly with clients, community partners, and internal staff for seamless operations.
- Ensure clients comprehend the role of Hospice Georgina and complete necessary documentation.

**Referrals and Case Management**:

- Evaluate referrals, redirecting appropriately to other community resources when necessary.
- Maintain accurate client data and manage necessary paperwork for each case.

**Reporting and Compliance**:

- Attend team meetings and provide feedback and data.
- Maintain comprehensive records of coordination activities and prepare summaries for board review.

**WORK HOURS**: Monday to Thursday, 8:30 am to 4:30 pm (with flexibility for evening and weekend availability as needed).

**Job Types**: Full-time, Fixed term contract

**Salary**: $25.00 per hour
