We are a dynamic community-based health centre promoting social justice and health equity in the Regent Park neighbourhood and east downtown area of Toronto. We operate out of several locations and provide a wide variety of services including: primary health care, chiropody, physiotherapy, health promotion, social work, community health, immigrant and refugee services, housing assistance, harm reduction and homeless programs, Pathways to Education for high school students, infant and child development programs, parenting education and family advocacy.

**Job Summary**:
The Community Health Worker (CHW) provides support to individuals who are experiencing homelessness and complicated mental health and substance use challenges, with an emphasis on those referred by the community health team, primary care and to also support the numerous walk ins. The CHW will also lead the _Health Promotion and Advocacy through Social Prescribing Project_ and will engage in health promotion activities and system navigation on site at RPCHC, to address and improve mental health issues for people experiencing homelessness and complicated mental health and substance use challenges. Through individual and group work the CHW will work to address social determinants of health with a human rights focus utilizing harm reduction, health promotion, community engagement, community development, social justice, trauma-informed and anti-racist/anti-oppressive (ARAO) strategies.

**This is a Full-time Contract position, until March 19, 2024. Staff will have the opportunity to participate in HOOPP (Healthcare of Ontario Pension Plan), receive benefits, and receive paid time off.**

**This is a bargaining unit position with OPSEU Local 5115.**

Individual Support - System Navigation:

- Provide targeted onsite support to individuals meeting the criteria for service
- Providing information and referrals, and service coordination to clients
- Utilizing low threshold and client centred strategies to assist clients to gain access to services and supports, onsite at RPCHC and externally
- Engaging in assessment and collaboratively developing intervention plans with clients
- Assist clients to connect to community supports, including specialized on site and off site programming
- Rendering crisis intervention in relevant situations and making referrals to other agencies if necessary
- Doing advocacy work on behalf of clients on an individual basis
- Monitoring and evaluating supports on an ongoing basis
- Completing appropriate documentation
- Providing accompaniments as needed

Community Capacity Building:

- Engage with this community to access appropriate resources around health, poverty and homelessness
- Develop on site programming that will work to improve mental health and reduce social isolation
- Develop monthly visits to Art Gallery of Ontario and Royal Ontario Museum
- Work collaboratively with the community, community partners and stakeholders to develop and implement action plans that address issues around health, poverty and homelessness

Advocacy:

- Develop a community led advocacy committee
- Work with other Regent Park Community Health Centre staff to improve the services we provide to homeless and under-housed people in our area
- Participate in systemic advocacy, grassroots organizing, awareness raising and public education for positive social change via involvement in community-based committees and networks
- Participate in coalitions and groups that seek to influence public policy and engage in advocacy and social action related to homelessness, harm reduction and other relevant social justice issues
- Develop and foster peer leadership through the advocacy committee

**Qualifications and Skills Required**:

- Post-secondary education in social work, community work, health promotion, adult education and/or equivalent in life experience
- A minimum of 3 - 5 years of community based experienced working with marginalized and homeless populations who experience multiple systemic barriers
- A minimum of 3-5 years proven case management and system navigation experience with people experiencing homelessness and systemic barriers
- Proven experience engaging in street outreach and low threshold models of engagement and support
- Extensive experience in conflict resolution and crisis intervention
- Program coordination experience, with a specific understanding of harm reduction, is considered an asset
- Extensive knowledge of overdose prevention and response strategies
- Excellent understanding of impacts of systemic oppression on marginalized individuals and communities
- Extensive knowledge of mental health, harm reduction and crisis services and systems
- Working knowledge of HIV/AIDS and HCV from a harm reduction perspective
- Awareness of and sensitivity to the health issues of a divers
