In accordance with Article 9.05, where the previous incumbent returns to this position within the thirty (30) day trial period this position shall be deemed not vacant and the posting will be cancelled. Should the vacancy be cancelled, applicants to this posting shall be notified.

**PRIMARY FUNCTION**:
The Lead Addiction Care Worker provides services in a manner consistent with Kingston Health Sciences Centre’s (HDH Mission, Vision and Values) and that reflects the core values of Patient and Family Centred Care (dignity, respect, information sharing, participation, and collaboration); works as a member of the Addiction Care team; collaborates and communicates effectively in a courteous and professional manner with clients, families and all members of the health care team of multidisciplinary professionals and support personnel; participates in departmental and hospital wide activities and programs that model and promote a safe and healthy patient-centred environment and that contribute to quality improvement and risk management initiatives; provides short-term, non-medical withdrawal management services to clients who want to detoxify from alcohol and/or other drugs, who are waiting for intake into a treatment program, or are in danger of relapse.

The Lead Addiction Care Worker has a key role in supporting the daily operation of the Detoxification Centre which may include assisting in completing scheduling, supporting staff with coverage to complete required learning modules, taking the lead on developing Standard Operating Procedures and obtaining input from staff, leading or championing quality improvement measures to improve patient care, and also acting as a key contact for internal and external providers to ensure timely and patient-centred care.

**PRINCIPLE RESPONSIBILITIES**:
**1. Patient Care**:

- Steps in to provide break and lunch coverage for ACW staff, and may need to:

- Ensure admission and discharge procedures, including search of clothing/belongings, are carried out according to established procedures and policies.
- Be alert to serious physical or mental health problems and, where appropriate, refer resident to the Urgent Care Clinic of Hotel Dieu Hospital or Kingston General Hospital Emergency Department, as well as collaborate within a multidisciplinary team (Nurse Practitioner, Nursing, Addiction Care Navigator, and Substance Treatment and Rehabilitation (STAR) Team)
- Interview all residents, to provide supportive counselling, to motivate residents to accepting referrals to ongoing community supports and to make appropriate referrals.
- Control and monitor entry of residents and visitors to the Centre.
- Conduct house checks every hour during the day, and conduct bed checks every thirty (30) minutes in order to detect seizures, hallucinations, etc. and to monitor the quality of sleep (from 0000-0630).
- Ensure that housekeeping activities are carried out, assist in completion and check that these have been properly completed when done by residents.
- Ensure that meals are prepared according to pre-planned menus.
- Review Refer Elsewhere List (REL) on a regular basis, and make updates when necessary.
- Review care plans as needed and update/communicate changes to front line staff

**2. Liaison**:

- Identify problems in occupational, health or fire safety and report these to the Program Manager
- Report any problems with unit appliances to Program Manager, and may be required to coordinate repair.
- Is a contact for community partners to reach out to help problem solve.
- Completing and monitoring service requests with corporate resources (e.g. IT, Maintenance, Facilities, etc.) in collaboration with Program Manager and Administrative Support

**3. Education**:

- Help staff complete any training recommended or directed by the Program Manager.
- Promote and Champion harm reduction strategies or other quality improvement strategies throughout the Centre

**4. Program Support**:

- Participate in continuous improvement opportunities to address barriers to care, patient and family concerns and/or issues related to house flow
- Support and participate in ongoing program development, analyzing opportunities for improvement in the system and influences changes to maximize patient care whilst minimizing waste.
- Working with the Program Manager to develop SOP’s and gain input from front-line staff.
