The Addiction Care Navigator provides services in a manner consistent with Kingston Health Sciences Centre’s Mission and Values (Compassion, Respect, Partnership, Excellence, Innovation) and that reflects the core values of Patient and Family Centred Care (dignity, respect, information sharing, participation, and collaboration); works as a member of the Mental Health Emergency Services Unit in the Emergency Department at the KGH-site; collaborates and communicates effectively in a courteous and professional manner with clients, families and all members of the health care team of multidisciplinary professionals and support personnel; participates in departmental and hospital wide activities and programs that model and promote a safe and healthy patient-centred environment and that contribute to quality improvement and risk management initiatives; provides support, advocacy and navigation of addiction services in the region to those that present in the ED, acts as an allied support for the STAR Team, helps support, coordinate and enhance the medical and therapeutic treatment of intoxicated patients and patients experiencing chronic substance use/abuse, promotes and champions harm reduction strategies, and demonstrates an awareness of and be responsible for actively promoting and supporting patient and family centered engagement and care in all we do.

**PRINCIPLE DUTIES & RESPONSIBILITIES**:
**1. Patient Care**:

- Monitor and support the patient’s journey from admission to discharge and transition into the community.
- Complete assessments on patients to provide supportive counselling, to motivate residents to accepting referrals to ongoing community supports and to make appropriate referrals.
- Communicate with patients, families and members of the interprofessional team to facilitate decision-making regarding care and transition to the community.
- Interact with patients and families to address questions or concerns they may have related to their care plan and their transition through and from the hospital.
- Collaborate with patients and their families and the interprofessional team to optimize recovery of the patient to the highest level of wellness possible, within an acute care setting.
- Obtain, monitor and document information to support a collective understanding of the care plan between the patient, family and the interprofessional team.

**2. Liaison**:

- Facilitate and monitor the coordination and mobilization of health care and community resources to meet the needs of patients and their families. Upon collaboration with the interprofessional team members, this may include referral and consultation.
- Assess and monitor the continuity of care in order to expedite and coordinate the delivery of services to facilitate the patient’s progress throughout the continuum of care.

**3. Education**:

- Assist patients and families in understanding their diagnosis and treatment options.
- Participate in the development of education materials for patients and families in Mental Health & Addiction Care.
- Participate in interprofessional education opportunities to increase knowledge and promote collaboration.
- Promote and Champion harm reduction strategies throughout the hospital with staff, physicians, patients and families.

**4. Program Support**:

- Participate in continuous improvement opportunities to address wait times, barriers to care, patient and family concerns and/or issues related to patient flow.
- Support and participate in ongoing program development, analyzing opportunities for improvement in the system and influences changes to maximize value patient whilst minimizing waste.
- Interpret and use the results of the patient satisfaction survey in order to report on the patient experience for continuous improvement initiatives

**QUALIFICATIONS**:

- Eligible for membership in the Ontario College of Social Workers and Social Service Workers (OSWSSW)
- 2-year diploma/certificate in the social services field or equivalent combination of education and experience.
- Minimum 3 years of recent related experience in Addictions, within the last 5 years required.
- BCLS Certificate at the Health Care Provider level.
- Successful completion of recognized non-violent crisis intervention program within previous 2 years.
- Practical experience in the field of social services.
- Must possess and demonstrate the skill and confidence to deal with persons in crisis situations.
- Enjoy working with patients who have addiction and mental health challenges to support and guide them in their recovery
