Position Summary:
This is a part-time contract - after school position (in person) that reports to the Site Success Supervisor. The Success Coach is responsible for utilizing leadership, advocacy, and collaboration to promote student success, provide preventive services, and respond to identified student needs by developing intervention strategies that address academic, behavioral, and social-emotional needs; and assisting with coordinating a wide range of activities to enhance program goals and objectives.

**Essential Duties and Responsibilities**:
A Success Coach maintains a caseload that does not exceed 35 elementary aged youth and delivers case management components that include, but are not limited to:

- Frequent contact with youth to administer behavioral/academic assessment and support tools:

- Psycho-social assessment to ascertain youth strengths and needs
- Individual Support Plan development to set personal and academic goals
- Positive Youth Development Inventory to determine their level of connectedness
- Youth Behavior and Life Choice Survey to assess risky behaviors
- Program curriculum designed to meet developmental needs and focused on key topics related to adolescent health and development, including building social, emotional, and life skills; developing a positive sense of self; and connecting with others
- Mentorship and guidance to ensure academic success and mental health support:

- Education achievement monitoring to identify strategies to effectively use their time and stay focused on their educational goals
- Support participants in building problem-solving skills to overcome challenges
- Bi-weekly review of grades, in addition to interim and report card grades
- Informal counseling to offer positive youth-adult interaction, guidance, and mentorship
- The Success Coach assists students make good choices and overcome obstacles
- In addition, Coaches work with students by providing suggestions on improving relationships with other students and friends
- Referrals to formal counseling are provided based on identified needs
- Provide group coaching in the form of education and skill building sessions on social emotional learning topics.
- Input data and maintain comprehensive case files for participants that document plan of care, and progress in accordance with set guidelines.
- Monitor and support participants in their progress toward goals as outlined in their Individual Service Plan (ISP).
- Work with youth and families to ensure they are receiving adequate support and continual involvement and engagement in so doing addressing any barriers to program participation.
- Maintains accurate and complete files for program participants as related to services and activities.
- Advises immediate supervisor of needs, trends and issues within area of responsibility and recommends activities to address them.
- Liaising between school-related personnel, including but not limited to, teachers, guidance, and behavior specialists, to identify barriers to academic success.
- Implement developmentally appropriate and prevention-oriented activities to meet student needs and school goals.
- Identify, observe and monitor students to evaluate areas of strength, weakness or special education needs, and refers families for support services.
- Cross collaboration with various departments, staff, families, district and outside support service providers including Speech, OT and ABA therapist.
- Complete family assessments, and organizing parent engagement events to support family strengthening activities
- Building rapport with families to connect them to community resources.
- Case notes to document interaction with youth and collateral contact are completed timely (within 24 hours).
- Participation in internal and community meetings as required, and in some instances, may also be required to represent the agency at community or social events.
- Substitute for absent Academic Success Tutors as needed.
- Professional and ethical representation of Patches 360 at all times.
- Other duties as assigned and needed for program support.

**Qualifications**
- Bachelor’s degree (preferably in Education, Social Work, Psychology, Sociology, or a related field)
- Minimum of three years’ professional experience working with elementary and middle school-aged youth and/or families
- Proficient in time management to fulfill all tasks in a timely manner adhering to program guidelines and supervisory expectations
- Must possess a proficiency working with Microsoft Office Suite and the ability to successfully navigate through the Internet, excel, power point, and other applicable software
