Client Care Coordinator

We are seeking a dedicated care coordinator to assist us with patients enrolled at our different facilities. You will be responsible for consulting with patients and determining their needs, developing care plans, coordinating patient-care services, educating them about their condition, and working with the care team to evaluate interventions.
The successful applicant will display a compassionate nature, be knowledgeable about health care practices, and provide exceptional customer service.
**Responsibilities**:
- Consult with clients and family members to discuss their care plan.
- Educate staff regarding the needs of the client.
- Working collaboratively with the client and medical professionals throughout the duration of the case to ensure high quality and clients/partner satisfaction
- Call-in on the patient regularly and evaluate and document their progress.
- Assist the care team with developing and assessing health interventions.
- Treat patients with empathy and respect and conduct oneself in a professional manner.
- Responsible for a variety of administrative tasks that evolve over time
- Comply with organizational guidelines and health care laws and regulations.
- Regularly schedule staff to client's requirements.
- Maintain a roster of active staff to deploy to new clients by screening and hiring qualified applicants.
**Requirements**:
- Problem solver - proactive and positive minded
- At least 2+ years' experience in care coordination, customer service, healthcare operations, case coordination/management, or other relevant
- Good verbal and written communication skills.
- Computer skills and proficiency in Google Workspace.
- Strong analytical thinking and the ability to handle multiple tasks concurrently.
- Excellent customer service.
- Compassion and empathy.
- A dedicated professional, with high level of responsibility and care for their work
