**Indigenous Care Coordinator**

**What you’ll do**
- Establish effective working relationships with community based agencies and health care providers who provide services for Indigenous communities to facilitate accessible and effective culturally safe, trauma informed client and family care.
- Using current best evidence, assesses advocates and provides recovery-oriented interventions to forensic patients and their families via culturally safe, trauma informed educational and therapeutic individual/group programs Maintains current knowledge and awareness of trends and activities in clinical practice including individual counselling methods, family therapy and group dynamics.
- Based within Rehabilitation Services, the Indigenous Care Coordinator connects with all treatment teams to advocate for and ensure the needs of self-identifying Indigenous patients are acknowledged and met though the care-planning process. Documents all patient related activities according to professional practice standards and BCMHSUS policies and procedures.
- Work in collaboration with other disciplines to develop and maintain a list of general and Indigenous community based agencies relevant to the needs of our clients, providing liaison services as required. Identifies clients' and family’s needs for services and makes appropriate referrals, or develops alternate strategies for client support, education and follow-up care.
- Promote a culturally sensitive and trauma informed approach to working with patients who self-identify as Indigenous through the fostering of supportive, collaborative therapeutic relationships. Prepare and participates in opportunities to educate patients, families, supporters, students, staff and community partners. Provide information to patients, families and communities regarding the provision and access to culturally safe, trauma informed mental health and substance use care programs and services.

**Qualifications**:

- Pursuant to section 41 of the British Columbia Human Rights Code, preference will be given to applicants of Indigenous Ancestry.
- Bachelor’s degree in a relevant Health or Social Sciences discipline.
- Two years related experience working in a health care environment that includes experience counselling individuals and families and group work or an equivalent combination of education, training and experience.
- Comprehensive understanding and experience delivering culturally safe, trauma informed care.
- Eligible for registration with designated professional regulatory body.

Skills & Knowledge
- Knowledge of federal, provincial, territorial, regional and Indigenous organizations mandated to provide mental health and substance use care services.
- Knowledge of and experience with Indigenous centered care and unique needs of Indigenous populations and health services, in alignment with Paige’s Story (2015) and the Truth and Reconciliation Call To Action report (2015).
- Knowledge of specific resources for Indigenous patients and families, i.e., First Nations Health Authority, Métis Family Services, Vancouver Aboriginal Child and Family Services Society and other delegated Aboriginal child and family services, Community Health Centre’s/Clinics on and off reserve, etc.
- Comprehensive knowledge of Indigenous history, cultures, communities, cultural interference, social determinants of health, and Indigenous health policy and direction
- Commitment to upholding the shared responsibility of creating lasting and meaningful reconciliation in Canada as per TRC (2015) and BC's Declaration on the Rights of Indigenous Peoples Act (2019).
