Job Summary:

 Under the supervision of the Program Manager, and as a member of a multi-disciplinary team, the Clinical Care Coordinator oversees the provision of nursing care and services in the Coast Transitional Cottages Program. The Coast Transitional Cottage (CTC) Program, funded by Provincial Health Services Authority, opened on the səmiq̓ʷəʔelə grounds (formerly Riverview) in 2003. In collaboration with PHSA’s BC Mental Health and Substance Use Services (), the CTC program provides psychosocial rehabilitation, clinical assessment, treatment, counselling and support to clients who are re-integrating into the community from the Forensic Psychiatric Hospital. We provide support for 41 clients in our nine cottages and two modular units, and outreach support to clients in semi-independent living apartments located in Coquitlam.

The Clinical Care Coordinator provides leadership, supervision and guidance to clinical staff and coordinates client services, ensuring the highest standard of nursing care. The coordinator consults, confers and collaborates with the program’s interdisciplinary team members, community treatment teams, the enhanced housing team and staff from Forensic Psychiatric Hospital to achieve excellence in client care with optimal health outcomes. Ultimately, the position is accountable for the overall effectiveness of clinical care in the program, in collaboration with client treatment teams. The Coordinator is responsible for coordinating the delivery of nursing care to clients, acts as a clinical resource to staff and assesses nursing staff professional development needs. The Coordinator, in collaboration with the program manager, oversees the development of procedures and practice protocols and is responsible for their effectiveness. The Clinical Care coordinator performs administrative duties by ensuring optimal use of resources within established budget restraints / limits.

The Clinical Care Coordinator assists in staff selection decisions, and in collaboration with program manager, conducts staff performance appraisals, The Coordinator practices in accordance with the standards of professional practice and code of ethics as outlined by the College of Registered Psychiatric Nurses of British Columbia (CRPNBC) or the College of Registered Nurses of British Columbia (CRNBC) as well as within a client centered, trauma-informed, strengths based, recovery-focused care model and the vision and values of the organization.

Requirements

-Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM) is required.

-Three (3) years' recent, related experience in the designated clinical area including one (1) year's related administrative/supervisory experience or an equivalent combination of education, training and experience.

-Valid BC Driver's License and access to personal vehicle for local area travel may be required.

-Valid First Aid & CPR

Knowledge and Abilities

Comprehensive knowledge of mental disorders, addictions and concurrent disorders and the treatment and principles of recovery in mental illness and addictions. General knowledge of the Forensic Psychiatry Act, Mental Health Act, Freedom of Information and Protection of Privacy act Comprehensive knowledge of Psychosocial Rehabilitation methodology, chemical dependency and addictions treatment, psychopharmacology (indications and side-effects) and psychotherapeutic and counseling skills. Demonstrated ability to provide effective leadership, supervision, work direction and consultation. Knowledge of evidence-based best practices and issues in the care and treatment of mental disorders, addictions and concurrent disorders. Demonstrated ability to develop and deliver educational materials for staff/clients and families based on education theories, principles and best practices. Demonstrated ability to promote a supportive, creative learning environment and continuous quality improvement. Demonstrated analytical, problem solving and conflict resolution skills. Demonstrated ability to plan, organize and prioritize work. Demonstrated ability to work collaboratively as a member of an interdisciplinary team. Demonstrated ability to set priorities, adjust to unexpected events, mediate and deal with conflict. Demonstrated ability to lead change and support staff through transition.
