The **Pathways for Change, Youth Mental Health Counsellor **position is responsible for directly supporting Strathcona County youth (primarily age 6-17 although this may extend to age 26) experiencing behavioral and mental health challenges. Using a strength-based, recovery-oriented approach that recognizes the importance of connecting with youth where they are at, this position assesses and implements comprehensive support plans that coordinate a range of community partners in the solution, as well as providing direct counselling to those not already connected to service or who are awaiting services. This position aligns with the Social Framework and is part of a Community Safety and Well-Being initiative bringing together RCMP, Enforcement Services, school divisions, Family and Community Services, and other key community stakeholders to support at-risk youth in Strathcona County. Using a team approach, this service provides coordinated and collaborative community-based supports known as Pathways for Change (P4C).

The P4C Youth Mental Health Counsellor is responsible for working directly with youth experiencing multiple at-risk factors such as limited community attachment, behavioral challenges, school difficulties, fractured family situations and are in need of connection and belong. Additionally, these youth may be experiencing mental health challenges. Utilizing a strength based, recovery-oriented approach that recognizes the importance of meeting individuals where they are at to coordinate and integrate services with a range of community partners to create a safe, supported and connected community for the youth.

**Responsibilities**:
Provide a full range of youth-centered services in various community settings for identified complex needs individuals:

- Assessing, along with intake team, the suitability for program participation and identify alternative supports or service. Utilize knowledge of psychiatric disorders in the assessment
- Utilize a strength based, wholistic, recovery-oriented approach to complete assessments
- Create suicide prevention safety plans and family violence screenings as required
- Providing mental health interventions and counselling support as part of treatment. Meeting with participants in person is preferred when safe to do so and the ability to be an effective practitioner through telephone and virtual platforms is needed
- Facilitating and participating in collaborative community tables for goal setting and case planning, supporting transitions to other providers, advocating for participant in other systems as well as completing referrals for additional services
- Meeting participants, when safe to do so, in various environments conducive and appropriate for case management and intervention support. This may include office, home, virtual, community, or other agency settings
- Providing guidance, consultation and education to other department staff and community partners as it relates to mental health and psychiatric disorders
- Engaging formal and informal support people to assist in ensuring safety, risk mitigation, and supporting intervention strategies that maintain stabilization. This includes consultation and liaison with families, community service providers, schools and police
- Ensuring department practice principles and values are reflected in service delivery at all times
- Effectively working with the supervisor for daily work assignments, check-ins, debriefs, problem solving, solution finding, feedback and effective communication. Day to day supervision is provided using a team approach that includes other members of the Human Services and Innovation team

**Adhere to case management practices that reflect the unique needs of each participant**:

- Engaging in regular supervision and caseload reviews, organizing and/or participating in collaborative case conferences
- Holding lead responsibility in collaborative communication with many different stakeholders and ensuring communication and information is shared in a timely and appropriate manner
- Ensuring that all documentation/assessments are completed professionally and in a timely manner
- Maintaining accurate information in the electronic record systems, such as NewOrg

**Develop and invest in strong working relationships with community partners and corporate partners, including Family and Community Services, to provide excellence in service delivery and enhance the continuum of care for individuals with complex mental health and/or social needs**
- Developing and maintaining strong collaborative relationships with the Family and Community Services team as well as many community partners including health services, Enforcement Services, RCMP, government agencies as well as several not-for-profit agencies
