**RESPONSIBILITIES**
- In collaboration with resident, staff and external workers, develop care plans and provide practical assistance
- Providing support related to housing, employment, financial aid, health care, social supports, and other areas as identified
- Supporting service users in building life skills; developing and facilitating weekly life skills and recreation activities
- Develop strong relationships with both current and new partner agencies
- Broker linkages to relevant community supports for service users
- Completing SPDAT, OCAN, OPOC, GAIN S.S. assessments and other assessments, as needed
- Submit accurate and complete records and data reports, as required
- Work collaboratively with partnering agencies and colleagues
- Engage in service user advocacy
- Provide frontline support and perform additional duties as required
- Assist with transportation and accompanying service users to appointments, as required.
- Assist with intake process for new residents.

**QUALIFICATIONS**
- Enrolled or completed a degree in Social Work, Psychology, Criminology or related field **or **at least 1 year working in a customer-facing role
- Experience working with, and knowledge of, resources concerning homeless populations, addictions, mental health, housing, and poverty issues is an asset
- Strong written and verbal communication skills
- Problem solving and organizational skills
- Strong interpersonal skills and ability to collaborate
- Computer proficiency
- Demonstrated ability to build rapport and work within the Harm Reduction framework, using NVCI principles is an asset
- Knowledge of case management principles and tools is an asset
- Demonstrated knowledge of Landlord and Tenant rights and responsibilities is an asset
- Must be legally entitled to work in Canada
- Must by at least 18 years of age
- Able to read text and communicate orally in English. Ability to communicate in other languages is considered an asset
- Ability to start working at the beginning of the contract and ability to work the full duration of the contract
- A recent vulnerable sector check completed at maximum 3 months prior to employment offer.
