**Overview**
We are seeking a compassionate and dedicated Community Care Worker to join our team. In this role, you will provide essential support and assistance to individuals in the community, helping them maintain their independence and quality of life. Your work will involve interacting with participants in their homes or community settings, ensuring they receive the care they need while promoting a positive and supportive environment. This position is 8:00AM-8:00PM on a two week rotating schedule. Average of 36-48 hours per week. Located in Spryfield.

**Responsibilities**
- Assist participants with daily living activities, including personal hygiene, meal preparation, and medication administration.
- Provide companionship and emotional support to participants, fostering a sense of well-being and connection.
- Maintain accurate records of participant care and report any changes in their condition to the appropriate personnel.
- Collaborate with healthcare professionals and family members to develop and implement individualized care plans.
- Participate in training sessions and ongoing professional development to enhance your skills and knowledge in community care.

**Experience**
- Previous experience in a caregiving or community support role is required.
- Strong interpersonal skills with the ability to communicate effectively with participants, families, and team members.
- Ability to work independently as well as part of a team, demonstrating flexibility and adaptability in various situations.
- A genuine passion for helping others and making a positive impact in the community.

**Clear up-to-date Vulnerable sectors and Criminal record check is required.**
