**Complex Centre for Diabetes Care**

Reporting to the Program Director, Chronic Disease Prevention & Management and Medicine Services, Thunder Bay

Regional Health Sciences Centre (TBRHSC), the Manager, Diabetes Care and Bariatric Medicine Programs will be

responsible for the efficient and effective daily operation of the Centre for Complex Diabetes Care, Bariatric Centre of

Excellence & Pediatric Healthy Living Program at TBRHSC; and Diabetes Health at SJCG.

The Manager will also have a matrix reporting relationship to the Director, Outpatient Rehabilitative Care & Chronic

Disease Management, St. Joseph's Care Group (SJCG) for responsibilities associated with Diabetes Health program

services.

Qualifications:
**Education**:

- Baccalaureate degree in a health related discipline or business administration from a recognized university,

Master's preferred

**Experience**:

- Relevant health care management experience ideally in Diabetes and/or Bariatric Medicine, demonstrating

progressive leadership responsibilities

Skills/Competencies:

- A proven ability to lead interdisciplinary care teams (Medical Lead, Physician, Nurse Practitioner, Nurse, Dietitian,

Social Worker, Kinesiologist, Pharmacist, Psychologist, Clerk, Indigenous Patient Liaison, Phlebotomist, Data Analyst

and Financial Analyst)
- Excellent communication skills, ability to deal effectively with patients and develop and maintain strategic

partnerships with health care service providers at the local, regional and provincial levels
- Extensive business and resource allocation skills, results oriented and demonstrated experience with clinical data

collection and quarterly/annual financial and statistical reporting including data quality, volume and costing data,

performance measures, wait time statistics and patient registries.
- Clinical service delivery, integration and promotion experience with proven ability to streamline care, promote

efficiency in service delivery, coordinate regional services, increase awareness and promote referrals to the

programs.
- Excellent problem solving skills, an ability to prioritize in a fast paced environment and to coordinate strategies to

engage and focus a team in process improvement, quality improvement and patient safety.
- Drivers license and reliable transportation is required

Salary Range: $54.409 to 64.011
