**Position Summary**:
The Social Worker provides assessments and services to meet the emotional and social needs for the residents and their families of Sherwood Care. Working collaboratively with other health care providers and the multidisciplinary team, the Social Worker provides direct psychosocial care, system navigation and advocacy. The Social Worker is responsible for providing and developing social work services to maintain or improve the health and social well-being and functioning of the residents. This is a **temporary** maternity leave coverage position (up to 18 months).

**Duties & Responsibilities**:

- Maintains a working knowledge of relevant medical/legal issues that impact on resident care
- Adheres to all regulatory requirements including, but not limited to, Alberta Health Services, Alberta Health, Continuing Care Standards, Accommodation Standards, Occupational Health and Safety and the Continuing Care Act
- Provides information and is a resource to residents and their families in preparing documents such as: Personal Directives, Enduring Power of Attorneys, Guardianship and Trusteeship
- Works with residents and their families to identify emotional, social and environmental strengths and problems related to their diagnosis, illness, treatment and/or life situation.
- Assists with the admission process
- As part of a team, formulates, develops and implements a plan of care for residents and their families, utilizing appropriate social work interventions
- Participates in and represents a social work perspective in multidisciplinary teams, rounds, meetings, etc
- Acts as an advocate and resource person for residents, family and staff.
- Assists residents and their families in accessing financial benefits

**Position Requirements / Skills & Abilities**:

- Diploma in Social Work
- Registered and in good standing with the Alberta College of Social Workers (ACSW)
- Minimum one-year relevant experience in a health care setting, preferably in the area of Gerontology
- Previous experience in a long-term care setting is preferred
- Must have a working knowledge of health care-related legislation and community resources and programs

**Job Type**: Part-time

Expected hours: 31.0 per week
