Provide individuals, families, and groups with the psychosocial and mental health support needed to cope with trauma, illness and lack of access to care. Services include advising family care givers, providing patient education and counselling, and making referrals for other services. May also provide care and case management or interventions designed to promote health, prevent disease, and address barriers to access to healthcare.

Job Task
- Collaborate with other professionals to evaluate patient’s medical or physical condition to assess client needs
- Collaborate with other professionals to assess client needs or plan treatments.
- Advocate for clients or patients to resolve crises.
- Refer patient or family to community resources to assist in recovery from Mental or physical illness
- Counsel clients in individual and group sessions to help overcome dependencies, recover from illness and adjust to life.
- Monitor clients to evaluate treatment progress
- Maintain clients’ records

**Job Types**: Part-time, Casual
Part-time hours: 20-30 per week

**Salary**: $31.00-$35.00 per hour

COVID-19 considerations:
Our center is currently adopting a hybrid model of care - half virtual and half in person visit.

All clients and staff are required to wear mask all the time.

We also telephone prescreening and another screening upon arrival.

**Education**:

- Bachelor's Degree (preferred)

Licence/Certification:

- Social Workers of Ontario Member (required)
