**Intake Worker - Part-Time (0.8) EFT - 1 year term with possibility of extension**

**$27.62 to $32.831 per hour**

Working as part of a multi-disciplinary team, the Intake Worker plays an integral role in the meaningful engagement of individuals and families with Youville services and supports and facilitating seamless supports across the organization and system.

Grounded in person-centered, culturally safe and social justice-oriented practice and a commitment to compassionate and responsive engagement, the Intake Worker works will all other team members to the reduce silos and system barriers many individuals face in healthcare.

The Intake Worker will build trusting and respectful relationships, carry-out thorough and timely assessments for eligibility to programming and services, assess the immediate intake needs of the people we serve, and provide information/resources as well as brief advocacy and system navigation.

The Intake worker will provide administrative supports to teams as defined by the programs and support outreach and community-based programming where identified.

**KEY RESPONSIBILITIES**

Engagement and Intake
- Builds effective and compassionate rapport and relationships with those seeking services/supports or programming from Youville.
- Provides initial assessment and screening of individuals contacting agency for service requests (based on established criteria of the program(s).
- Assesses eligibility for services and supports at the diabetes education centre
- Receives referrals, matches individuals to practitioners, services and/or supports, including referral to other agencies/services if appropriate
- Manages and processes wait lists
- Ensures all appropriate intake documentation is completed.
- Provides back up support to individuals utilizing community and drop in services.

**Administrative Support**
- Prepares and maintains statistical data, correspondence, reports and other documentation as required.
- Assist with front reception duties, including telephone coverage, appointments, taking payments and general customer service as needed.
- Compile letters and other communication as it relates to intakes, referrals and advocacy

**Crisis-Intervention and De-Escalation**
- Evaluate immediate safety risks, de-escalate and diffuse conflict in an assertive, timely fashion

**Qualifications**:

- Completion of high school
- Minimum 5 years’ experience in social services environment performing eligibility/intake assessment
- Health Care experience an asset
- Knowledge and experience (personal or professional) in diabetes an asset
- BSW preferred
- Demonstrated capacity to build effective and “safe” rapport/relationships
- Extensive knowledge of community resources and health and social services system
- Demonstrated ability to work within multi-disciplinary teams
- Demonstrated ability to work with diverse populations and those who are systematically oppressed.
- Excellent communication skills
- Demonstrated capacity in crisis intervention and de-escalation
- Demonstrated bilingualism is an asset (French and English or English and another language)

**Additional Requirements**:

- Criminal record check required
- Child and Adult abuse registry checks required
- A valid driver’s license
- Fluency in both French and English an asset
- Availability to work evening hours as required (1-9pm)
