Pickering, Canada The West Durham Family Health Team Full time

The primary focus of the full-time Clinical Social Worker is to restore, maintain and enhance the emotional and social functioning of the diverse patient population in the Durham region. This is a hybrid position with the incumbent **working two days per week remotely**.

The Social Worker works in collaboration with an interdisciplinary team including physicians, nurse practitioners, nurses, care coordinator, dietitian, pharmacist and occupational therapist.

The Social Worker uses a **brief solutions-focussed counseling model** to provide patient support. The Social Worker provides various types of interventions including individual and group counseling, and system and care navigation. The Social Worker participates in the development, implementation, monitoring and evaluation of programs and services provided to patients of the WDFHT.

**Qualifications and Experience**:

- Master’s Degree in Social Work
- Current certification with the Ontario College of Social Workers and Social Service Workers
- Minimum three years’ experience as a Clinical Social Worker; experience in a primary care setting preferred
- Skill and experience in individual and group psychoeducational sessions including cognitive behaviour therapy, self-management, and Mindfulness.
- Excellent crisis intervention skills
- Knowledge of community resources and familiarity with community outreach
- Strong ability and interest in linking patients to community resources
- Effective interpersonal and communication skills (written/verbal), self-directed.
- Superior problem-solving and conflict resolution skills
- A strong team player with the ability to prioritize, manage time effectively and be flexible in a dynamic work environment.
- Ability to be innovative and creative in order to develop strategies that successfully meet the needs of diverse patient population.

**Roles and Responsibilities**:
The role and responsibilities of the Social Worker in the Family Health Team include but are not limited to:

- 1) Assessment_: Assessment and diagnosis of psychosocial problems
- 2) Treatment and Management_:

- Provides **individual and group psychoeducational services**;
- Works within the primary care model of brief therapy and ensures seamless transition to community resources as appropriate; and
- Facilitates group counseling sessions including modalities such as cognitive behaviour therapy, self-management, motivational interviewing and Mindfulness.
- 3) Case Management and System Navigation_:

- Provide case management and system navigation, including facilitating linkages to appropriate services, supports and community resources.
- Knowledgeable about community and the resources available, especially in the areas of Crisis services, Mental Health and Long-term care.
- Assists the referring physician/health care providers with referrals to secondary and tertiary health care centres
- 4) Patient-Centred Programs:_
- Involve patients in decisions about their own health, promotes and support patient self-management
- Communicate effectively with patients, team members and external agencies involved in the patient circle of care.
- 5) Education/Advocacy_:

- Provide assistance in navigating service delivery networks to find required resources, and advocate for patient access to needed resources.
- Collaborate on the _design and delivery of patient education programs_.
- Develop and deliver health promotion and psycho-education related to the prevention of mental health problems.
- Assist in _monitoring and evaluating_ the success in achieving program objectives.
- 6) Administration_:

- Collect and document accurate and comprehensive patient-related information in the Electronic Medical Record (EMR) including patient history, assessments, interventions and care plans.
- Prepare quarterly service reports for Ontario Health.
- 7) Teamwork and collaboration:_
- Work with team to develop collaborative approaches for mental health services in primary care.
- Actively participate in staff, team and program development meetings as appropriate

**Job Types**: Full-time, Permanent
