QUALIFICATIONS:
Baccalaureate level of post-secondary education in Nursing (RN) or Social Worker (BSW/MSW)Post graduate certificate in geriatric and/or psychiatric nursing preferredMember in good standing with the College of Nurses of Ontario (CNO) or the College of Social Workers and Social Service Workers (OCSWSSW)Minimum 3 years recent related clinical experience with the geriatric populationDemonstrated sound knowledge and understanding of community mental health client and care giver service needs and treatment modalitiesExhibit thorough understanding of theory and practice of geriatric medicine and psychiatryCompetency in Standard corporate software (Microsoft Office - Word, Excel, Outlook), capabilities in data entries and searchesExcellent interpersonal and organizational skillsStrong time management skills and ability to communicate effectivelyCritical thinking and problem-solving skillsProfessional verbal and written communication skillsAbility to work independently and collaboratively as part of a multi-disciplinary team and other agencies in the communityBasic understanding of all relevant legislation including PHIPA
RESPONSIBILITIES: (Duties include, but are not limited to)Maintains a clinical caseloadLiaise and provide consultation, treatment, follow up and discharge planning; review and evaluate the treatment and ongoing care in consultation with the client and caregiver/care providerMonitor treatment modalities for efficacy and adverse reactions including pharmacological and non-pharmacological interventionsComplete comprehensive client centered geriatric physical, functional, and cognitive assessmentsMonitor and document client’s progress and response to therapeutic interventionsParticipates in team activities related to clinical care (e.g. statistics) and functionParticipates in activities related to program/organizational development, community/client educationParticipates in clinical coverage dutiesParticipates in research within scopeInvolved in group or other treatment modalities as appropriate to patient population and skill levelSupports, precepts, and aids colleagues, students and others to provide evidence-based best practice clinical service excellenceThis community position provides clinical assessment and support in and across all communities in Haldimand NorfolkOther duties as assigned
