The Systems Navigator provides intake services for new clients, and provides a comprehensive assessment of their strengths, capacities and needs, based on the social determinants of health.

They will assist clients in navigating the broader health care system while also establishing and maintaining a strong relationship with community partners.

This position offers a unique opportunity to make a difference in the lives of newcomers and refugees, and those significantly marginalized by poverty, addiction, mental health, and housing issues.

This role will work with our newcomer clinic, supporting newcomers to Canada along the continuum of care within a health promotion framework and liaising with interdisciplinary team members and external care providers to facilitate the best plan of care for clients.

REQUIRED QUALIFICATIONS

  • Postsecondary education in social services or related health care discipline
  • Minimum 3 years of experience in community, health and/or social services working with highly vulnerable people
  • Ability to assess and evaluate a client's health and social needs, as expressed by the client
  • Experience in fostering and maintaining strong relationships with community partners
  • Extensive knowledge of communitybased services and resources
  • Knowledge of trauma and violenceinformed care and experience working with individuals experiencing trauma
  • Understanding of and commitment to health equity
  • Awareness and understanding of the social determinants of health
  • Ability to work with marginalized people of diverse backgrounds with an inclusive and antioppressive lens

PREFERRED SKILLS

  • Excellent communication skills, both verbal and written
  • Strong organizational and time management skills
  • Ability to prioritize competing demands
  • Strong computer skills, including the use of MS Office Products and Electronic Medical Records (Telus Practice Solutions Suite currently in use)
  • French and other language skills are considered an asset in supporting the communities we serve
