**KEY FUNCTION**:
Process referrals, book appropriate appointments, and provide program information in accordance with Ontario Health (Cancer Care Ontario) High Risk Ontario Breast Screening Program (OBSP) guidelines, policies, and pathways. Support data collection for quality reviews and perform chart reviews of the North East OBSP sites. Support the clerical staff at OBSP sites with the use of the clinical information system.

**REPORTING**:
Under the general direction of the Regional Coordinator, Quality, Performance Management & Reporting.

**DUTIES**:

- Receive and process High Risk OBSP referrals, liaise with diagnostic imaging staff and leadership and perform follow-up with radiologists, primary care providers and genetic counselling as required.
- In accordance with CCO and NECC standards, book patients for mammograms, Magnetic Resonance Imaging (MRI) and ultrasound tests and enter appointments into Integrated Client Management System (ICMS) and Meditech.
- Explain the high risk breast screening process to the patient, answer general questions and confirm that the patient would like to participate in the program.
- Communicate bookings to patient and primary care provider and send results and follow-up correspondence.
- Book follow-up imaging all abnormal screening results in accordance with the reading radiologist direction.
- Enter results into database, generate and send result letters to patient and referring Primary Care Provider.
- Track patients and ensure annual recall.
- Access data from Northeast Ontario hospitals, access Electronic Medical Records (EMR) of other hospitals, request faxed patient results and enter all data into ICMS.
- Perform quality assurance of OBSP data collection and reporting and develop Standards of Work as required.
- Liaise with the Regional Breast Imaging Lead (RBIL) and Regional Medical Radiation Technologist (RMRT) regarding specific patient care or program issues as required.
- Track that affiliate sites have completed data entry for each month in a timely fashion.
- Coordinate and perform chart audits and flag issues to the Coordinator or Manager.
- Verify and reconcile data ensuring correct internal case counts.
- Collect and provide draft reports for OBSP quality indicators.
- Facilitate new ICMS user registration and rights assignment, providing training and support to North East ICMS users as required and respond to queries regarding data entry, OBSP processes and procedures.
- Problem solve ICMS report queries and develop/run reports for specific data requests; prepare quarterly OBSP regional metric reports as required.
- Participate and supports regional OBSP meetings.
- Assist with program promotion activities as required.
- Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
- Participate in hospital training as required.
- Identify issues, plan improvements and measure success of improvements as required.
- Perform other duties as required.

**QUALIFICATIONS**

**EDUCATION AND TRAINING**:

- Minimum of a one (1) year Diploma in Medical Office Administration from an accredited college.
- Current certificate in Medical Terminology is required.
- Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.

**EXPERIENCE**:

- In-depth knowledge of the Ontario Breast Screening Program, Integrated Client Management System is required.
- Experience working with a clinical information system both as end user and for data mining and reporting purposes.

- Demonstrated knowledge of and proficiency with medical terminology.

- Demonstrated superior interpersonal and communication skills, both written and verbal.

- Ability to manage multiple tasks and to prioritize tasks.
- Demonstrated ability to effectively interact and communicate both orally and in writing with patients, families, physicians and other healthcare workers.
- Demonstrated intermediate computer skills with advanced proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems such as ICMS and Meditech.
- Proficiency in data entry.
- Demonstrated compliance with HSN’s privacy policy.
- Demonstrated commitment to the safety of co-workers and patients.
