JOB TITLE: Case Manager, Housing First Worker

POSITION LEVEL: Experienced; non-manager

POSITION TYPE: Full Time salary; 3-month probationary period

HOURS: 40 hours/week; Monday to Friday 8:30 a.m. to 4:30 p.m.; with some flexibility, including 1 afternoon shift/week

JOB OVERVIEW: To oversee case planning and provide comprehensive support to their specific caseload while ensuring implementation of agency philosophy, policy and procedures. The Housing First program provides housing supports and services to vulnerable youth, specifically from the BNL using the coordinated entry system. The Case Manager will help youth to develop and pursue their housing goals, and will support them to identify, access and navigate service systems relevant to their specific and individualized needs.

ESSENTIAL DUTIES AND RESPONSIBILITIES
- Providing youth with a continuum of care that includes case management, referrals and outreach in Guelph and Wellington County
- Liaising with relevant community partners to provide coordinated care
- Complete intake assessments to gather background information and for assessment
- Assist them with creating an effective treatment plan tailored to their specific needs
- Provide crisis intervention and support to clients in the program
- Support life skills development through programming and individual intervention
- Collaborate with community partners to provide wrap-around support
- Develop relationships with clients to support emotion regulation, harm reduction and life skills development
- Liaise with staff to coordinate care and routines and support with behaviour management and treatment goals
- Provide or support programming related to skills development, recreation and therapeutic goals
- Provide trauma-informed and client-centered care
- Support youth through their involvement with the Youth Justice System, including court, lawyers, mental health diversions and probation
- Write case notes, intervention plans, treatment plans
- Plan case conferences and review meetings with relevant community partners
- Support with data collection and reporting to funders
- Attend committee meetings and conduct presentations within the community as required
- Identify community services and resources that are appropriate for clients’ needs, coordinating referrals, intakes, appointments and on-going access to services
- Occasional coverage support to staff sites
- Provide AHCS 2 weeks per year

JOB SKILLS AND QUALIFICATIONS
- Post-Secondary education in Social Services or related field (BSW preferred)
- 2+ years’ experience working with vulnerable youth with complex needs
- 1 Year experience providing case management and/or support to vulnerable youth
- Sound knowledge of the barriers facing street-involved youth and commitment to the principles of housing-first
- Working knowledge of the Mental Health Act, Youth Justice Act, and the Child and Family Services Act as they pertain to work with high-risk youth
- Skills, knowledge, and attitude consistent with trauma-informed client-centered advocacy and care
- Demonstrated crisis management, conflict resolution, problem solving and priority setting abilities
- Demonstrated knowledge of local social services and the processes by which to engage them
- Demonstrated understanding and sensitivity to social and cultural diversity, including ability to support clients’ decisions without judgement or bias
- Strong organizational, communication, interpersonal and crisis intervention skills
- Strong ability to problem solve and work independently
- Background facilitating skill development training or recreational coordination is considered an asset
- Experience with staff supervision and support or program coordination is considered an asset
- Crisis Intervention and/or A.S.I.S.T. training is considered an asset

MANDATORY REQUIREMENTS
- Child and Youth Worker diploma or related degree
- CPR / First Aid level C
- Criminal Reference Check, including vulnerable persons
- Covid Vaccine x2
- Valid Driver’s License and access to a reliable vehicle, regular travel to rural areas is required

PREFERRED
- Bachelor’s degree in social work or working towards
- Registered with the College of Social Workers and Social Service Workers
