**Clinical Coordinator**:
The Clinical Support includes follow up with youth who have been housed access basic needs and services. You will work in collaboration with the staff and external partners to facilitate the monthly clinical calendar, ensuring there is a broad range of internal and external clinical programs to help support the youth in the development of life skills so they may successfully transition into independent living. This position is also responsible for the overall maintenance of the Case Management Program.

**Key Responsibilities**:

- Assisting clients with client advocacy and individual case management support
- Develop and implement community development initiatives
- Cultivate partnerships with various community professional and agencies.
- Implement Residential Tenancy Act
- Administer procedures regarding social welfare, employment, child welfare, immigration and refugee law
- Work in collaboration with other Programs and management
- Building relationships, rapport, and providing emotional support to clients.
- Working co-operatively as part of a team. Team expectations include regular meetings, planning events and other related activities.
- Available to provide on call support on a rotating basis.
- Duties and responsibilities may require early morning and evening hours; there for flexibility in work hours is required
- Assessing clients’ issues and needs in order to assist, support, and advocate with or for them in obtaining basic needs.
- Preparing and monitoring case planning goals.
- Keeping appropriate case notes and entering data.

**Qualifications**:

- S.W,B.A.,B.S.W, or relevant education and experience in social work, social services, community development, or related field
- At least two years’ experience working with individuals with mental health/substance abuse issues, and from culturally diverse backgrounds
- Demonstrated experience in advocacy, case management framework, crisis intervention and prevention, conflict resolution and negotiation skills
- Strong interpersonal, organizational, verbal and written communication
- Proficient in MS Office, including Access, Excel
- Able to maintain a positive approach and a professional manner at all times
- Possess Crisis Intervention, First Aid & CPR and relevant development training
- Understanding of cultural competency framework
- Ability to work effectively as part of a team and interact with staff of different departments
- Available to work flexible hours including evenings and weekends
- Valid G driver’s license
- Clear Police Check and Vulnerable Sector Search must be maintained.
