**DESCRIPTION**

**The Spiritual Health Practitioner (SHP) works collaboratively within interdisciplinary teams to provide patient and family centered care that enhances and facilitates the spiritual health for patients and/or family members. Through clinical assessments of psycho-social-spiritual and existential needs and provision of clinically appropriate interventions the SHP enables patients and family members to use their psycho-spiritual health resources to face their given circumstances throughout their hospitalization. The SHP provides situationally appropriate support for staff; educational opportunities related to spiritual health provided to physicians, staff, interdisciplinary teams, learners, volunteers; and participates in organizational initiatives.**

**PRIMARY RESPONSIBILITES & DUTIES**:
Clinical Practice
- Accept referrals and consult requests from physicians, interdisciplinary teams and self-referrals from patients and/or family members.
- Assess the psycho-social-spiritual and existential distress and strengths of the patient and/or family member (respectful of their unique spiritual, religious, cultural traditions/needs and world views) that enable the collaborative development of spiritual health goals during the patient’s hospitalization.
- Provide psycho-social-spiritual health interventions to help the person meet their spiritual health goals, whether immediate or over time during their hospitalization, and makes referrals to physicians and interdisciplinary team members or to community resources when appropriate.
- Ensure clinical practice is informed by emerging and evidence based practices.
- Assist when appropriate with the communication of values and decisions between patients, family members and interdisciplinary team members.
- Help to facilitate healthy communication and attention to values, decisions and care specific to dying and death, including life legacy.
- Participate in interdisciplinary team consultations and rounds to assist the team in the holistic care they provide for specific patients and/or family members and to communicate the spiritual health needs of the patients and/or family member(s).
- Document clinical sessions in the PHI record.
- Enter workload measurement data in Emerald.
- Facilitate connections between patients and family members through the use of video technology.
- Provide psycho-social-spiritual support through video technology.

Staff Support
- Provide situational - focused support for staff members and learners in relationship to complex clinical situations that cause moral distress and spiritual distress.

Education about Spiritual Health
- Provide formal and informal education about the nature of spiritual health, emerging and evidence based practices for spiritual health assessments and interventions, and the scope of practice of Spiritual Health Practitioners to professionals and students of other health care disciplines.

Organizational Initiatives
- Participate in organizational initiatives (e.g. Quality Improvement) where appropriate.

Spiritual Health Team
- Participate in the Spiritual Health team to assist with effective functioning of Spiritual Health services in the organization, quality improvement initiatives, program development, and collaboration with Spiritual Health services at Providence Care, HDH and other initiatives as identified.

**QUALIFICATIONS**:

- Completion of a Master's degree program in Theology, Psycho-spiritual Therapy or Spiritual Care
- Certification with the Canadian Association for Spiritual Care (CASC)
- Registered with the College of Registered Psychotherapists of Ontario (CRPO)
- At least one (1) year of relevant clinical experience
- Effective organization, interpersonal and communication skills
- Knowledge of applicable, legislative, KHSC and /or departmental policies
- Service oriented, with the ability to effectively work with diversity and appreciate that people with different opinions, backgrounds and characteristics bring richness to the challenge or situation at hand
- Excellent counseling/clinical skills and compliance with confidentiality requirements
- Exercise initiative and good judgment with ability to prioritize and multi-task
- Demonstrated competence in enabling effective team collaboration between colleagues, staff, volunteers and community resources
- Ability to relate theoretical foundations to the field to spiritual health clinical practice
- Proven ability to attend work regularly.
- Satisfactory criminal reference check and vulnerable sector check required.
