**Job Summary**:
The SLFNHA Developmental Services provides services to the 31 communities in the Sioux Lookout First Nations Health Authority catchment.

Under Developmental Services this position provides support to the interdisciplinary team (Audiology, Psychology, Fetal Alcohol Spectrum Disorder (FASD), Rehabilitation) who will provide assessment and deliver rehab services. This work will be done through a clear referral process. This position will establish working linkages and service protocols with communities and organizations that provide health/social services programs. This position will participate in orientation and be the support for the clinic staff.

The Community and Clinic Coordinator is directly accountable to the Developmental Services Program Manager.

QUALIFICATIONS:

- Bachelor’s Degree in Human Services or equivalent education and minimum 2 years experience
- Experience and understanding of Anishinaabe cultural issues, geographical realities and social conditions with the northern remote first nation communities
- Community contacts in the northern remote communities
- Knowledge of NIHB travel requirements and Jordan’s Principle
- Experience with FASD or rehab clinic coordination
- Case management, assessment and service planning skills; specifically holistic approaches to service delivery
- Ability to work effectively with multiple disciplines with a knowledge of available local and regional resources
- Advanced written and verbal communication skills
- Demonstrated organizational and time management skills
- Word processing and database management skills
- Ability to work independently
- Knowledge of Anishinaabe language an asset.
- Drivers license is required.
- The ability to perform the requirements of the position on a regular basis.

ROLES AND RESPONSIBILITIES:
1. Facilitate access to children’s rehabilitation services by providing case management and service coordination for children, youth, and their families/caregivers by providing general information, determine eligibility and priority for service by using priority standards set out by Developmental Services and the rehab services, arrange travel and provide follow up as needed.
2. In consultation with Developmental Services is responsible for the day to day functions of the Sioux lookout clinics including scheduling clinic times, clients to be seen, coordination of all travel, file management and all other activities around the
3. To work with the First Nation communities for the direct community visits including organization of Band Council resolutions that are required, organizing all travel, client scheduling and community education.
4. Assist with completing intake packages as necessary.
5. Compile summary reports post clinic and maintain tracking system for all clients seen through clinics, discharges and follow up required.
6. Ensure high standards of professional conduct and quality of care.
7. Travel to remote communities required.
8. Any other duties assigned.
