**Housing Case Manager, Sudbury East**
- **Department **:Community Development & Housing
- **Employment Status**:Full-Time, Permanent
- **Hours per Week**:35 hours per week. May include weekday/weekend/evenings
- **Work Location**:Works independently out of community housing buildings in St. Charles, Warren and Noelville.

**RESPONSIBILITIES**:
Reporting to the Coordinator, Housing Case Management and working in partnership with the Manitoulin-Sudbury District Service Board (MSDSB), the Housing Case Manager provides supports to individuals residing in community housing units operated by the MSDSB. Other duties include:

- Build rapport and assist with tenancy-related issues
- Provide support to individuals with the development of goal-focused care plan and wellness strategies
- Provide direct, hands-on support to individuals in maintaining their living environment and assistance with instrumental activities of daily living skills
- Develop educational programs relevant to identified need
- Ensure all file management and reporting functions are maintained in comprehensive and timely manner

**QUALIFICATIONS**:

- Completion of a 2 year diploma or degree in a health or social services discipline.
- Combination of post-secondary education and 5+ years of experience that reflects the job description and requirements of the current position will also be considered.
- Minimum one year experience in related position.
- Access to a reliable vehicle required to complete job duties
- Proficiency with Microsoft Office software required
- Knowledgeable of community agencies and resources
- Professional Designation/Certification (one of the following required

a. Registration, in good standing, with a regulating professional body Ontario College of Social Work and Social Service Worker (e.g.OCSWSSW); or

b. Certification, in good standing, with a professional certifying organization* within health or social service fields e.g. Certified Psychosocial Rehabilitation and Recovery Practitioner (CPRRP) from Psychosocial Rehabilitation (PSR Canada); or

c. Working towards obtaining one of the above within 6 months of hire
- *Organization must have a written code of ethics, core values and standards that are consistent with those of CMHA-S/M. Certification must require members to meet a core set of competencies, have a requirement for continuous learning to maintain certification and be approved by CMHA-S/M._

**SALARY**:
**Bachelor Degree**
- Annually, Step 1: $48,718.97 - Step 4: $57,316.43
- Hourly, Step 1: $26.77 - Step 4: $31.49

**College Diploma**
- Annually, Step 1: $43,942.59 - Step 4: $51,697.17
- Hourly, Step 1: $24.14 - Step 4: $28.41
