**ROLE TITLE**: Rapid Rehousing Supportive Community Worker

**PLACEMENT**: FT Contract, 40 hours per week for 18 weeks

**GENERAL FUNCTION**:
Reporting to the Rapid Rehousing Team Lead, this position will be part of a multi-faceted team that oversees the creation and operation of safe, affordable, hopeful communities of housing and shared spaces across a spectrum of affordability, location, size, permanence, and support. Supportive Community Workers, alongside Housing Workers, the Clinical Support Lead, and various community partners, will provide an intensive, housing-focused program, moving people from homelessness to permanent housing which meets their needs and goals within an accelerated timeline. They will work closely with residents at our Rapid Rehousing Centre to create spaces for lives to intersect, connect people with local support services and agencies for professional assistance, provide practical and emotional support on a day-to-day basis, and develop hope through housing.

**RAPID REHOUSING SUPPORTIVE COMMUNITY WORKER-SPECIFIC DUTIES & DETAILS**:

**Supportive Community**
- Create opportunities for people to connect, create, and contribute
- Work alongside Clinical Support Lead to assist residents in implementing individualized plans to support their holistic care (mental, physical, and emotional well-being)
- Assist residents in identifying personal goals, actions, and timelines and work toward them
- Help residents connect with community resources
- Support connection to mental health and wellness supports, provide encouragement, coaching conversations, and support with self-care, as well as facilitating access to onsite and community-based counselling services
- Support connection to physical health and wellness supports (medical appts, methadone)
- Create an individualized Safety Plan with each resident
- Work within a team comprised of staff, volunteers, community partners, and peer support to build relationships and develop long-term networks of support
- Work with team to plan and facilitate bi-weekly house meetings for information, community building, and dispute resolution
- Various administrative duties including responding to inquiries for supported housing, managing and facilitating donation requests, creating a monthly calendar, and contributing to social media platforms and monthly update communication
- Work with the Volunteer Coordinator to coordinate and manage volunteers and volunteer opportunities
- Daily HIFIS entry
- Data entry (case notes, incident reporting)
- Engaged participation in interdisciplinary support with residents and community partners
- Attendance and participation in staff meetings
- Contribution to the continued analysis and adjustments of systems and processes to ensure Redwood’s ongoing provision of safe, affordable, hopeful housing and supportive community

**Housing**
- Prepare units (coordinate repairs, touchups, and maintenance; create warm, welcoming space with appropriate furniture, food, bedding, etc.)
- Facilitate move-in (access instruction, key/keycard distribution, and/or setting entry codes, tour of facility including laundry, garbage, mail, community spaces, and neighbours)
- Work with the Property Team to facilitate monthly unit visits for fire safety and maintenance
- Work with Housing Team to support residents with implementing their Housing Plans and achieving their housing goals
- Assist with transition from Rapid Rehousing Centre to permanent housing, including assistance with giving notice, packing, planning, referral to Furniture Bank services, and cleaning
- Work with the team to conduct a wrap-up interview with each resident to seek input regarding our premises, services, and supports

**QUALIFICATIONS**:

- Post-Secondary Degree or Diploma in Social Services, Sociology, Psychology, Counseling, or related field (post-secondary students will be considered)
- Additional degrees and/or certifications is a plus (Level C First Aid, NVCI)
- Lived experience and/or working experience in communities we support is an asset
- Experience working with vulnerable populations, may be exposed to various types of escalation
- Extensive knowledge of the non-profit space, particularly in the context of affordable housing and the Housing First strategy
