Reporting to the Professional Practice Leader, Clinical Nutrition, the Dietitian II, Diabetes, utilizes relevant experience and advanced skills to design, implement, evaluate and document the appropriate patient nutritional care plan. Uses advanced clinical techniques and patient counselling/education skills in the area of diabetes management for both individuals and groups. Participates in the development of clinical nutrition practice guidelines and educational materials for Diabetes, and acts as a resource person in Diabetes in accordance with professional standards and program objectives. Participates in practice-based research and knowledge translation projects in collaboration with the health care team.

**Education**:

- Bachelors degree in Food and Nutrition or Dietetics from a recognized University.
- Dietetic practice education from an accredited program meeting the Integrated Competencies for Dietetic Education and practice (ICDEP).
- Maintains certification as a Certified Diabetes Educator.
- Two (2) years recent experience working with patients in an acute care setting, including experience in diabetes nutrition management and patient education, or an equivalent combination of education, training and experience.
- Current registration in the College of Dietitians of B.C.

**Duties**:

- Evaluates nutritional status/risk of patients/clients with diabetes and designs, implements, evaluates, and documents an appropriate nutritional care plan for patients with diabetes and related health concerns
- Utilizes principles of learning and adult education to provides comprehensive nutrition, lifestyle counselling and self-care management strategies for patients/clients through one-to-one and group teaching sessions
- Works collaboratively with the interdisciplinary team to provide guidance to clients on specialized insulin regimens by providing guidance on adjusting insulin around food intake, activity and for blood glucose correction.
- Works together with the Clinical Nurse Leader and the interdisciplinary team to triage patients coming into the Diabetes Centre to provide the most appropriate care and education for the patient.
- Identifies nutrition related patient educational needs for the diabetes area; initiates, prepares and presents educational programs and materials in collaboration with regional, departmental, program and other team members.
- Acts as a resource person in diabetes for other dietitians and the interdisciplinary team by providing support for handling complex cases within PHC and other outreach programs.
- Identifies continuing education needs in the diabetes area and organizes education opportunities for dietitians and other health care professionals within PHC and other outreach programs by reviewing professional literature, consulting with and providing advice for health professionals, initiating, developing and presenting relevant educational programs for dietitians and other health care professionals.
- Participates in quality improvement activities by initiating, planning, developing and evaluating quality improvement activities/nutrition interventions/programs including audit tools, written procedures, standards and clinical guidelines in the diabetes area for Clinical Nutrition Services and relevant clinical programs.
- Supports clinical research activities by identifying areas of research, assessing current research relevant to diabetes, generating questions and participating in writing proposals, collecting and interpreting data and dissemination information stemming from the research findings within departmental resources.
- Assists with dietetic practice education by identifying learning objectives and enabling activities, and providing clinical education and supervision to dietetics students
- Contributes to an interdisciplinary approach to patient/resident care and program related issues by attending ward rounds and team conferences to exchange information related to patient care and discharge planning.
