Summary: Reporting to the Professional Practice Leader, Clinical Nutrition, the Dietitian II for Urban Health utilizes relevant experience and advanced skills to design, implement, evaluate and document the appropriate patient nutritional care plan. Initiates and develops clinical nutrition practice guidelines and educational materials, and acts as a resource person for other health care providers around the province, in accordance with professional standards and program objectives. Initiates practice-based research projects in collaboration with the health care team.

Qualifications / Skills and Education: Education, Training and Experience

Bachelor’s Degree in Food and Nutrition or Dietetics from a recognized University. Dietetic practice education from an accredited program meeting the Integrated Competencies for Dietetic Education and Practice (ICDEP). Two (2) years recent experience working in a medical/acute care setting with one year working in the areas of addiction, HIV/AIDS, mental health and food insecurity or an equivalent combination of education, training and experience. Current registration in the College of Dietitians of B.C.

Duties and Responsibilities: 1.Evaluates nutritional status/risk of patients/residents including complex cases in designated practice area. Develops/designs, implements, evaluates (initial and ongoing), and documents an appropriate nutritional care plan. Liaises with food service staff regarding patient food services related concerns.

2.Instructs and/or counsels patients and family members in nutritional issues such as diet modifications and lifestyle changes. Facilitates group education sessions when appropriate.

3.Determines, refers and/or liaises with community based nutritional resources as required to ensure that patient care issues are addressed and coordinated.

4.Completes required patient/resident documentation, forms and reports and documents workload statistics.

5.Acts as a resource person for dedicated area of practice for other dietitians and other health care professionals, by providing support for handling complex cases within PHC and other outreach programs for designated practice area.

6.Identifies continuing education needs and organizes education opportunities for dietitians and other health care professionals within PHC and other outreach programs by reviewing professional literature, consulting with and providing advice for health professionals, initiating, developing and presenting relevant educational programs for dietitians and other health care professionals.

7.Participates in quality improvement activities by initiating, planning, developing and evaluating quality improvement activities/nutrition interventions/programs including audit tools, written procedures, standards and clinical guidelines for Clinical Nutrition Services and relevant clinical programs.

8.Supports clinical research activities by identifying areas of research, assessing current research relevant to area of practice, generating questions and participating in writing proposals, collecting and interpreting data.

9.Identifies nutrition related patient educational needs for area of practice; initiates, prepares and presents educational programs and materials in collaboration with regional, departmental, program and other team members.

10.Assists the Professional Practice Leader with the practicum planning for students and dietetic interns by identifying learning objectives and enabling activities, and providing clinical education and supervision in area of practice.
