A challenging and exciting opportunity exists to work in a collaborative inter-professional environment delivering an innovative rehabilitation service designed to help patients with stroke or other neurological conditions and their caregivers reach their optimal level of recovery. This work assignment is shared between the Community Stroke Rehabilitation Team (CSRT) and the Comprehensive Outpatient Rehabilitation Program (CORP). CSRT is a community-based program consisting of an interdisciplinary team that provides specialized rehabilitation to stroke survivors in a home and community setting. This role will entail liaising, collaborating and fostering close relationships with community partners such as Dale Brain Injury, Life After Stroke programs and March of Dimes Canada.
