**About the Job**:
Reporting to the Program Manager, Central Intake, the Community Mental Health Counsellor, Wait List Service Navigator, is responsible for supporting applicants and their families on the wait list for services and supportive housing at SHIP.

Their main goal is to ensure that applicants receive consistent follow-up and service navigation while they remain on our waiting list for services and/or supportive housing at SHIP. They will also develop and facilitate group programming as well as deliver Single Session Therapy (SST) to applicants on the wait list through the OPEN (**O**pportunity for **P**riority and **E**ffective **N**avigation) program.

**Duties and Responsibilities**:

- Conduct clinical reviews of files on the service and supportive housing wait lists to determine any changes to service needs
- Partner with applicants and their families as members of the health care team
- Use standardized assessment tools within a community setting
- Ensure appropriate records are kept and ensure that statistical information including client demographic data is maintained as per agency standards
- Deliver Single Session Therapy (SST) for the OPEN program
- Facilitate group programming for wait list applicants
- Service co-ordination, follow-up and advocacy
- Develop and maintain a community resource network
- Refer to partnering organizations and provide linkages to community supports through system access navigation
- Provide streamlined transitions with “warm transfers”
- Complete evaluations and feedback surveys with applicants and/or their families
- Provide comprehensive, holistic, person and family-guided access to services
- Work independently and collaboratively within a multidisciplinary team to carry out mental health and substance use recovery support
- Understand and be able to integrate a philosophy of person-guided care into daily responsibilities
- Demonstrate a strong commitment to quality care and service delivery based on SHIP’s Mission and Values
- Adhere to agency Health & Safety Program and the Client Safety Framework in the promotion of client safety
- Perform additional duties as required/delegated by Management
- Work towards creating inclusive and equitable spaces using an anti-racist and anti-oppressive approach
- Use a health equity approach to identify and dismantle inequities within our systems
- Demonstrate ability to uphold PHIPA legislation
- Excellent written and verbal communication skills
- Strong interpersonal and organizational skills
- Be self-directed and reliable to achieve consistent goals
- Profound knowledge and experience working with a diverse population of varying levels of education, professions, and personal life experiences from a broad range of cultural and socio-economic backgrounds
- Able to multi-task and work well under pressure in a fast-paced environment

**Qualifications**:

- Post-secondary education in Social Work, Social Services, Health Services, or equivalent
- 3+ years experience providing assessments on individuals with mental health challenges and single-session therapy delivery
- Registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) is an asset
- Registration with Inform USA/Canada is an asset
- Demonstrated knowledge of serious mental illness, concurrent disorders, and/or dual diagnosis, and experience with intake services preferred
- Excellent assessment, risk management, and crisis intervention skills
- Expertise in collaboration with healthcare services
- Knowledge of MS Suite (Word, Excel, Power Point and Outlook)
- A vehicle, valid driver’s license, and adequate insurance coverage
- Valid Certification in First Aid & CPR
- Training in ASIST and CPI

**Reporting Function**:
The CMHC, Wait List Service Navigator reports directly to the Program Manager, Central Intake. They work with both internal and external partners and will interact with all levels within the organization.

They are a support system, resource, and an advocator for the team and Organization.

**Working Conditions and Physical Requirements**:

- Hours are typically from Monday - Friday, 8:30am - 4:30pm but may vary depending on client needs
- Average amount of physical effort is required
- Able to sit/stand for prolonged periods of time
- Able to answer and communicate telephone calls and general computer use
- Able to lift up to 20lbs (product, documents, etc.)
- Vehicle travel to sites for visits/walk-throughs and open doors as required.
