The West Nipissing General Hospital was built in 1977 and boasts a clean, modern facility, with over 100 beds in operation at any given time. The Hospital operates a 24/7 Emergency Department serviced by General Practitioners, visiting Surgeons, Anesthetists, and Internists.

The West Nipissing General Hospital hosts regular cardiology, urology, nephrology, otolaryngology, endoscopy, surgical lumps & bumps and psychiatry clinics and consultation visits by specialists. There exists an excellent referral relationship between West Nipissing General Hospital and the Regional Health Centre in North Bay and Health Sciences North in Sudbury. The West Nipissing General Hospital is the sponsoring agency for a community Mental Health and Addictions Program.

The West Nipissing General Hospital collaborates with the Ontario Telemedicine Network to provide consultations and follow-up visits with specialists. In addition, Ontario Telemedicine Network hosts numerous education events for both staff members and physicians. The West Nipissing General Hospital offers a satellite Oncology program through the Regional Cancer Program of Northeastern Ontario Regional Cancer Centre. Chemotherapy services are provided to the residents of West Nipissing

**Job Description**:
The West Nipissing General Hospital is currently seeking a temporary part-time Discharge Planner to join our team. Reporting to the Manager of NCU/CCC, you will be responsible for coordinating multidisciplinary efforts to identify and work with residents, patients and families requiring assistance with discharge, advocacy, linkages with community agencies and providing brief supportive counselling.

**Job Status**: Temporary, Part-Time

**Hours of work**: Days, 37.5 hours every two weeks.

**Work Location**: On-site

**Salary Range**: $41.58 to 43.70 per hour, plus an additional 14% in lieu of benefits and 6% vacation pay

**Qualifications**:
**Education & Experience**:

- Bachelors of Science in Social Work, Nursing or related field
- Current registration with affiliated College
- Related experience an asset
- Management experience in a health care environment is an asset

**Skills Required**:

- Superior interpersonal skills with the ability to establish relationships while exercising maturity, tact, confidentiality and discretion
- Ability to function effectively in a highly dynamic environment, including working under pressure, adapting and responding to changing priorities and meeting deadlines
- Strong communication skills with effective team-building and leadership abilities
- Demonstrated proactive approaches to problem-solving with strong decision-making capabilities
- Excellent organizational and time management skills
- Computer proficiency is a requirement
- Bilingualism (French & English) is an asset
- Up to date Immunization is required including COVID-19 Vaccinations
- Excellent attendance record
