**Role Overview**

We are seeking dynamic and innovative individual who will work enthusiastically to deliver recovery oriented, client and family centered and comprehensive services that meet client needs, ensure client safety and work within a quality improvement framework to identify opportunities for enhancing quality and quantitative outcomes of the services. Client and family centered care is guiding principle at Cota that helps improve decision making processes, health outcome and client experiences. The successful individual will embed this guiding principle into their work.

**About Team**

The Toronto West Integrated Service Team is comprised of Case Managers who have expertise in a variety of areas including: Acquired Brain Injuries, Mental Health, and Mental Health.

**Summary of Duties**:

- To work collaboratively with internal and external partner agencies to provide short term CM care for clients 65 years and older with some exceptions.
- To network with external agencies to obtain relevant resources for clients care and to make referrals to Retirement Homes and Long Term Care facilities as if needed.
- To support/encourage clients to participate in community activities to build social network.
- To advocate with and on behalf of clients to improve and/ or maintain wellbeing.
- To create, maintain and improve healthy working relationships with the client and their circle of care.
- To work collaboratively with their team members to support all clients on the TWIST team.
- To attend and engage with others on the Cota Geriatric Community of Practice meetings and network with other Geriatric case managers.
- To be accountable for meeting benchmarks and completing documentations on time.
- To contribute to the overall growth and wellbeing of the Team.
- To keep up with relevant knowledge and information to better support the clients.

**Summary of Qualifications**:

- A preference for a Bachelor’s degree in a relevant discipline (i.e. Psychology, Social Work, etc.), AND
- One year of experience in a position working with the Geriatric Population.
- OR, other relevant education and experiences that is determined to be acceptable by the interviewers
- Knowledge of Dementia and other age
- related illnesses is preferred.
- Knowledge of Diversity Equity and Inclusion issues and Intersectionality.
- Ability to model Cota’s core values with clients, co-workers and partner agencies.
- Knowledge and understanding of the client population and an ability to work effectively with people living with serious mental illness respectfully.
- Demonstrated strong commitment to the rights and ability of each person with a serious mental illness to live successfully in community residences, work in market jobs, and have access to helpful, adequate, competent, and continuous supports and services.
- Knowledge of principles of case management theory and practice using a client-centred and mental health Recovery philosophy.
- Excellent clinical judgement and decision-making skills including a keen awareness of service users-clinician boundaries and ethical dilemmas.
- Experience working on a multidisciplinary team is an asset.
- Experience working in a community agency is an asset.
- Experience working with external organizations and building positive working relationship.
- PIECES certificate training is an asset.
- Knowledge of Microsoft Office Suite (Word, Excel, etc.).
- Excellent knowledge of community resources and/or demonstrated ability to search for applicable resources/information.

**Other Requirements and Working Conditions**:

- Visits will be conducted in the participants’ homes, clinics, hospital, shelters, and other community settings as required and require staff to travel-by vehicle drive to get there.
- Availability to work day & evening hours, and weekends as may be required.
- Role adjustment as needed.
- Access to a vehicle.
