Role Overview

  • The Case Manager
  • Mental Health_ _is a dynamic and innovative position, that will deliver recovery oriented, client & family centered, and comprehensive services to meet client needs, ensures client safety and works within a quality improvement framework to identify opportunities for enhancing quality and quantitative outcomes of the services.

About the Team

  • ARCS is a hospital diversion program in partnership with North York General Hospital and Humber River Health. The objective of this program is to reduce emergency department visits, by providing shortterm, community based case management services to individuals that had recently visited the emergency department for mental health and/or addiction matters.
  • The ARCS team is interdisciplinary, consisting of Mental Health Case Managers, Addiction Specialist Case Managers, a Peer Support Specialist, and an Intake Case Manager.

Primary Responsibilities:

  • Initiate services to clients at the point of intake to ensure that proper assessment and initial service response occurs.
  • Have excellent knowledge of community resources and/or demonstrated ability to search for applicable resources/information.
  • Develop coordinated care plans with/ for clients and complete the Ontario Common Assessment of Need (OCAN) and other assessments as required.
  • Collaborate with other systems, organizations and individuals from courts, probation, shortterm residential beds program, hospital and community to ensure a wellcoordinated service system.
  • Record client status/progress using the appropriate documentation system.
  • A combination of a postsecondary degree in a related field and 2 years direct experience, assessed as being relevant to the specific target population being served.
  • Cognitive Behavioural Therapies (CBT) and Motivational Interviewing an asset.
  • Knowledge of principles of case management theory and practice.
  • Sound clinical knowledge and understanding of the client population and demonstrated ability to work independently and effectively with the mental health population.
  • Demonstrated ability to use problemsolving techniques to identify and assess client needs and develop appropriate intervention strategies.
  • Demonstrated ability to engage in a meaningful relationship in a short time with a broad range of clients, including those who are resistant.
  • Skillful in assertive outreach and followup using a flexible approach to support/assist clients in remaining engaged.
  • Ability to facilitate within 3 months the transition of clients from crisis to an increased level of autonomy to promote stability and empowerment; recognizing when the client is ready for discharge or referral.
  • Excellent clinical judgment and decisionmaking skills including a keen awareness of clientclinician boundary issues.
  • Excellent knowledge of community resources and demonstrated ability to negotiate with others on behalf of clients.
  • Excellent written and verbal. communication and interpersonal skills.
  • Excellent organization skills.
  • Experience in working within an interdisciplinary team is an asset.
  • Knowledge of and commitment to an Anti-Racism Anti-Oppression framework as well as an ability to articulate and implement Recovery Philosophies.
  • Proficient in MS Office, Pirouette and OCAN documentation.

Working conditions:

  • Must have access to a vehicle.
  • Conduct visits in the clients' locations, including but not limited to homes, shelters and hospitals.
  • Regular onsite involvement within Humber River Health's Inpatient Psychiatric Unit.
