Join our new multi-disciplinary team of passionate and committed professionals working to support clients with complex mental health issues and/or substance use disorders, and have come into contact with the law. This Rehabilitation Assistant position will support the Enhanced Care Housing Project.

The Enhanced Care Housing Project supports clients found not criminally responsible or unfit to stand trial due to a mental disorder as they transition out of the Forensic Psychiatric Hospital and into the Community Transitional Care (CTC) housing program located on the səmiq̓ʷəʔelə lands (former Riverview Hospital Lands, Coquitlam). The CTC program provides recovery and rehabilitation services to facilitate a safe and successful transition and reintegration of forensic patients into the community.

This position will be temporarily based out of the Surrey Forensic Regional Clinic; but will later transition permanently to the səmiq̓ʷəʔelə lands.

This role is ideally suited to a person who enjoys supporting clients to achieve success with reintegration in their community, working in a team environment, solving problems, and working collaboratively with partners. This person must be committed to upholding the shared responsibility of creating lasting and meaningful reconciliation in Canada as per TRC (2015) and DRIPA (2019).

**What you'll do**
- Work collaboratively with the interprofessional team to develop/implement individualized rehabilitative treatment plans for patients by identifying patient strengths, limitations and functional level for program involvement. Work with team members in the regular evaluation of assigned programs with regard to patients’ assessed needs and progress, and modifies or changes programs as indicated, e.g., recommending suitability for programs. Utilize skills and knowledge to assess a patient’s condition and make in the moment adjustments to interventions in response to the patients current health status.
- Complete rehabilitation assignments within designated scope of practice and documents patient progress. Report observations of patients’ behavior and progress to therapist and Treatment Team. Communicates concerns and/or changes to patients’ condition. Prepare individual discharge reports to meet department/organization documentation standards at the conclusion of each closed program by summarizing information focusing on patients’ life skills and independence levels and recommending appropriate community supports where available. Maintains outcome and workload measurement statistics.
- Work collaboratively with other Rehabilitation colleague, therapists and Treatment Team members to analyze the needs of target patient populations as a basis for new program development, standards and evaluation strategies by reviewing relevant literature and suggesting interventions that will support the achievement of desired treatment outcomes. Participate in the development of specific program curricula to address the functional level of the target population including researching, obtaining, and creating appropriate educational, testing, and other program materials to meet established program goals and objectives and evaluation processes under the guidance and supervision of the designated therapist
- Maintain work area and equipment by ensuring equipment and supplies, such as tools, materials and chemicals used in the program are stored appropriately and accounted for, arranging repairs as required, and assuming the responsibility for the purchase of supplies to support programs as required. Cleans, disinfects and maintains equipment including mobility self-care aids and adaptive devices, and reports malfunctioning equipment to appropriate staff. Fabricate/repair adaptive devices such as splint straps under the direction of the designated therapist.

**What you bring**
- Completion of a Rehabilitation Assistant Program or an equivalent combination of recent, related education and training plus 2 years clinical experience.
- Food Safe Certificate
- Valid Class 5 BC Driver's License
- Ability to communicate effectively both verbally and in writing
- Physical ability to carry out the duties of the position
