JFSC is expanding and recruiting in our Older Adults Community Outreach Team

**Responsibilities and Accountabilities**:

- Liaise and advocate with natural and professionals supports.
- In partnership with clients, develop a service plan and goals.
- Access resources and tools to improve individual well-being, aging in place, emotional support and advocacy.
- Actively participate in collaborative service delivery and system navigation with agencies participating in The Way In Network, including carya, Calgary Chinese Elderly Citizens Association, Calgary Seniors Resource Society and 211.
- Communicate with community and healthcare professionals regarding client’s needs and progress.
- Measure clients' progress regularly and adjust services accordingly.
- Assist with program development and implementation aligning with best practices.
- Increase client knowledge of community programs and support, in effort to reduce isolation.
- Ensure all case notes align with practice policy.
- Maintain current knowledge and skills in professional designation, in accordance with role requirements.
- Knows and adheres to the formal and informal departmental goals, standards and policies and procedures.

**Qualifications**
- Degree/Diploma in Social Work as well as registration as RSW
- A minimum of 3 years working with individual/older adults
- Certification in Suicide Intervention, First Aid/CPR and Non-Violent Crisis Intervention would be an asset
- Must have exceptional interpersonal and communication skills and the ability to collaborate and build strong networks with a range of community stakeholders and service providers
- Familiarity and aptitude to learn new software and information systems
- Valid Drivers License with reliable vehicle
- Clear Police Security check

Pay: $55,000.00-$58,000.00 per year
