Your Opportunity:
Located at the Sheldon M Chumir Health Centre, under supervision of the Calgary STI Clinic & Safeworks Outreach Program Manager, this position provides a range of support services to clients accessing both community and Alberta Health Services resources/services and assists in overcoming access barriers to STBBI services and care. This position also participates in the provision of health promotion and disease prevention programs and provides consultation and assistance to the health care team regarding service provision. The role also includes delivery of safer injection and safer inhalation supplies, health promotion and education, and referrals to other health & social services. The position is may also assist clients to appointments, liaising with community and AHS programs, and supporting the multidisciplinary team. The Outreach Worker is responsible and accountable to the public for their own practice according to the standards defined by their professions’ college as applicable. The Outreach Worker maintains competent practice and provides care according to their professional regulations.

Description:
As an Outreach Worker, you will work as a member of a multidisciplinary team, supporting clients to stabilize in their current environments as an extension of their clinical care service. As an Outreach Worker, you will conduct outreach assessments, determine outreach service goals and develop outreach treatment plans in collaboration with clients and clinicians. You will facilitate client connections with community and government supports and advocate for clients with these external service providers. You will focus on establishing housing, financial, food and clothing, transportation and social supports for clients. You will be responsible for providing education and coaching support related to daily living skills and activities (attending appointments, money management, searching for jobs, grocery shopping, etc.), observing and reporting changes in the clients' behaviours, participation and ability to function independently to the interdisciplinary team. You will evaluate client progress and functioning and make adjustments to the outreach treatment plan as required. As a part of your role as an Outreach Worker, you may be required to provide outreach crisis stabilization support for clients, exercising various de-escalation techniques and linking clients with the appropriate supports (EMS, Police and Crisis Team, etc.). You also may be required to enter clients' residences and assist clients to transfer in and out of vehicles, care facilities and residences.

**Required Qualifications**:
Completion of diploma from a post-secondary program in a related field.

**Additional Required Qualifications**:
Minimum 1-3 years’ experience working with diverse and marginalized populations. Experience/lived experience in creating safer spaces for all people including all sexual orientations, gender identities & expressions, those who identify as people of color, Indigenous people, people with disabilities, or newcomers to Canada. Computer proficiency with complex programs and databases; documentation is completed in the Electronic Medical Records. Ability to manage complex clients with mínimal supervision. Ability to think independently and make decisions in a fast-paced and demanding work environment. Excellent attendance record & reliability. Excellent communication skills (both written and verbal) demonstrated through previous employment records & references. Ability to independently support a patient to the ground or sustain a patient in Trendelenburg position (lift/carry 30+ lbs. - waist to floor, floor to crown). Ability to handle periods of intense activity and multi-task throughout high-volume clinics/events, including walking up to a distance of 8 km per shift. Ability to sustain periods of stationary activity. Ability to independently perform the skills of a certified BLS provider. Knowledge and understanding of the Truth & Reconciliation Commission Calls to Action.

**Preferred Qualifications**:
